Affiant believes the property and evidence described above is evidence of the crime
or crimes of Criminal Homicide
Aggravated Arson
Obstruction of Justice.
The facts to establish the grounds for issuance of a Search Warrant are:
Your affiant has received training and certification from the Utah Peace Officers
Standards and Training Academy. Your affiant has over 35 years of police
experience, including 23 years in investigations. Your affiant is currently assigned
to the Salt Lake City Police Homicide Squad and has the responsibility to
investigate homicides that occur in and around Salt Lake County.
1. On May 22, 2016 at 02:23 the Salt Lake City Police and Fire Departments were
notified of a fire at 574 North East Capitol Street in Salt Lake City.
2. First responders found the house engulfed in flames. A man was calling for help
from the top level of the home.
3. Fire Department personnel found John Williams deceased on the fourth floor.
4. The Fire Department noted suspicious details and called out arson
investigators. Arson investigators responded and verified the fire was an arson
where an accelerant was used. The arson investigators searched the home for
evidence.
5. A search warrant was served on 574 North East Capitol Street on May 22, 2016.
The eWarrant number for that warrant is 1450204. After the warrant was served,
the house was released to John Williams family.
5. Laura Fosgren is the niece of John Williams. She is an attorney and is the
executrix of John Williams estate. She is married to Matt Fosgren.
6. On May 22, 2016 Matt Forsgren entered 574 North East Capitol Street after the
police had finished processing the crime scene.
7. Matt Fosgren entered the house with members of his family. Matt went to the
fourth floor using an exterior ladder and a spiral staircase. He entered the fourth
floor master bedroom to retrieve family photographs and property not destroyed
by the fire. He discovered wooden matches in the master bedroom.
8. The wooden matches were lying on a surface that was covered in soot. The
matches are clean and not covered in soot. It appears that the matches were
placed after the fire had been extinguished and after law enforcement left the
scene.
9. The matches were pristine and not burned. The matches were not covered with
soot, as was all the rest of the bedroom and property inside the bedroom. Your
Affiant talked to Matt Forsgren on 5/23/2016. Mr. Fosgren emailed your Affiant
a picture of one of the matches in the master bedroom. Your Affiant observed a
clean unburned wooden match on a surface covered with soot.
10. Your Affiant prays for an order from this court to enter 574 North East Capitol
Street and search for the unburned matches and any other evidence of the crimes

YOU ARE THEREFORE COMMANDED:
to make a search of the above-named or described person, vehicle, item, and/or
premises for the herein-above described property or evidence and if you find the same
or any part thereof, retain such property in your custody subject to the direction of a
prosecutor or an order of this Court.
I declare under criminal penalty of the State of Utah that the foregoing is true
and correct.
