We have just received your notification for your immigration medical exam.
Your medical exam (simple check up with X ray) with the French immigration authorities (OFII) is
scheduled on Thursday, April 7, at 9:30 am, and will take place at the following address:

You will find attached your notification for your appointment.
The day of your medical exam, you should have with you, at the minimum:

The attached notification/convocation letter from OFII.

Your original passport.
1 passport sized photograph (4.5 cm x 3.5 cm French official standard see guidelines
attached you may have it done in an automatic photo booth and there are some in some
major food shops, major metro stations, city halls, police stations, etc.).
A less than 3-month old proof of domicile in your name (an accommodation statement from
your Residhome please email us its scanned copy first for our review).
your glasses if any.
58 Euros to be paid with fiscal stamps (timbre fiscal in French, which are available in some
of tobacco shops).

The day of your appointment, the OFII should affix a stamp in your passport. This stamp will confirm
that every necessary immigration formality has been accomplished and that you are legally
authorized to reside in France for training purposes, and up to November 26, the expiry date of your
entry visa.
Please do not forget to provide us with the scanned copy of every item that you will receive after
your immigration medical exam (that should be your medical exam certificate and the OFII stamp), so
we can verify that everything is in order.
