Tooth

restorations are the different type of ways to


remove the decayed portion of the tooth and then
repair that section of the tooth. The more the tooth
decays the more severe the treatment is needed

chose this because this is a fundamental part of


dentistry. Learning about this topic will help me
explore more deeply into dentistry which will let me
know if this is truly something that I want to be.

Tooth Restorations Continued

Types of fixed tooth restorations:

1.

Fillings: the most basic type of tooth restoration that deals with minor
cavities that cannot be remineralized. The teeth are filled with material to
cover the cavity.

2.

Crowns: a tooth shaped cap that restores the size and shape to a tooth. This
is used when there is a more serious cavity.

3.

Bridges: bridges are fake teeth which hold together missing teeth by putting
a crown on either side to anchor the missing tooth to the working teeth.

4.

Implants: an implant is a piece of metal that is put into the bone socket like
a screw in wood to stop anymore decay or to fill in the missing area. A fake
tooth is attached to the top.

Crowns

A crown is a tooth shaped cap that is placed over a tooth to cover the tooth
to restore its shape and size, strength, and improve its appearance.(http://

Crowns can be used to:

Protect a weak tooth

Restore a broken tooth

Hold a dental bridge in place

To make a cosmetic modification

Ceramiic

These types of crowns are made purely from ceramic

This material usually is used because of its appealing look and how well it
blends in with the rest of the teeth

The two types of ceramic crowns I am focusing on are zirconia and E-max

A zirconia crown is a durable, easy to wear, and long lasting ceramic crown

Advantages of zirconia include the strength of the material, the aesthetics or


attractiveness of the tooth, along with the need for minimal tooth preparation.

An E-max crown is a ceramic crown that is preferred for its long lasting and
aesthetic qualities

This type of crown is considered a good option for damaged, stained, or poor
quality teeth

Gold Alloys

Gold crowns are usually very durable being able to withstand heavy forces
from chewing. They do not chip or break, making them last a long time.

Gold crowns also cause low wear on the opposing tooth.

Gold alloy crowns are usually used to restore molars and premolars because
of their color.

Gold alloy crowns are one of the classic types of dental crowns.

Gold crowns are somewhat out of date because of materials like zirconia and
E-max that have coloring that resembles the tooth.

Base Metal Alloys

Base metal alloys consist of traditional non-noble metals with high strengths.

This material crown is often fitted into back teeth.

Advantages of this crown consist of:

Long lasting

Cheaper than ceramic crowns

Disadvantages of the crown is that the crown is prone to chipping and


fracture along with aesthetic issues to having a silver colored crown.

Additionally ceramic is fused to the metal to add aesthetics.

Crown Process

The process of creating a crown is usually a two visit process consisting of:

1.

Assessment of the tooth(first visit)

2.

Preparation of the tooth(first visit)

3.

Fitting and cementing the final tooth (second visit)

Assessment of the Tooth

When the patient first arrives x-rays of the tooth may be required to check
the roots of the tooth receiving the crown. If the tooth has excessive decay or
if there risk of injury to the tooths pulp, a root canal may be needed prior to
the crown.

Preparation of the Tooth

After the dentist has decided that the crown is needed, anesthetics will be administered to the nerve.
The anesthetics is 2% lidocaine with 1:100,000 epinephrine

Then an impression will be taken. Alginate material will then be squeezed into the impression tray.
Afterwards the dentist will place this tray into the mouth of the patient allowing for the patient to
bite into the impression. This will create a mold of the tooth that needs the crown along with the
proximal teeth.

Next the crown is filed down to create room for the crown. The amount that the tooth is drilled into
depends on the type of crown being used.

After reshaping the tooth the digital scan impression must be taken.

Next the alginate impression will be used to create the temporary crown. Next place the integrity
material into the alginate impression.

The impression will then be placed back onto the teeth that were used for the making of the
impression. The integrity will harden within 71 seconds. Then the impression will be removed leaving
a tooth shaped cap over the existing tooth. This temporary crown will protect the tooth along with
maintain the appearance of the tooth while the permanent crown is being created at the lab.

Fitting and Cementing of the Tooth

At the start of the second visit, the dentist will remove the temporary crown
and check the fit and coloring of the permanent crown.

The dentist will then numb the area using anesthetics.

The permanent crown will then be cemented to the tooth.

Depending on the type of crown used the crown should last between 10 to 15
years.

Fillings
A

filling is a way to restore a tooth damaged by


decay back to its normal function and

The

filling is used when the decay of the tooth can


not be remineralized or when a tooth is cracked.

Fillings

are designed to close off areas that bacteria


could possibly enter to prevent further tooth decay.

Amalgam Filling

An amalgam filling is the traditional type of filling that consisting of a mixture


of metals

Amalgam fillings are also known as silver fillings because of their silver
appearance.

This is a good material because it is long lasting, works well under pressure,
and is relatively inexpensive

Amalgam fillings are out of date due to the composite filling

Composite Filling

Composite filling is a tooth colored filling that has been developed in the
recent decades

These fillings provide good durability and resistance to fracture

Composite fillings are the ideal filling for front teeth because of their
aesthetic qualities.

The disadvantages to composite fillings are the increased cost of the filling
material. Along with this amalgam fillings are generally more durable.

Steps to Create a Filling

The dentist will first assess the tooth using x-rays to determine if a filling is needed on the
tooth.

Next the nerve will be numbed using anesthetics.

Once the tooth is completely numbed, the dentist will remove the decayed material from the
tooth using a high speed drill.

Once the decay is removed the dentist must shape the cavity in order to have the right shape
to be able to take the filling.

Next the tooth will be etched using a gel composed of 38% phosphoric acid. The enamel will be
etched for 15 seconds while the dentin will be etched for 30 seconds. This will allow for tiny
holes in the tooth enamel surfaces which will allow the filling to bond tightly with the tooth.

After the tooth has been etched the bonding material will be applied to the tooth. Then a blue
curing light will be used to harden the bonding material.

Finally the filling will be packed into the cavity and hardened using the blue light.

Conclusion

Going through this class and getting this mentorship experience has taught me
a lot this year. I have learned that beyond a doubt I want to be a dentist and
will continue to pursue this. I have gotten to learn about the different types
of equipment, procedures, and patient care that goes into being a dentist. I
have had to learn to be more responsible and organized when trying to keep
up with all the work that this class requires. Overall I think I am a better
person coming out of this class. I hope that some day I can make this dream
of being a dentist a reality some day. I thank all the people that have helped
me along through this journey and have helped me better myself through this
experience.
