AFFIDAVIT OF DISCREPANCY FOR CORRECTION OF DATE OF BIRTH
I, JERSON GRINGCO GUMOBAO, of legal age, Filipino, married and a
resident of 258 Jade St. Sison Subd, Buhangin, Davao City, Philippines, after
having been sworn to in accordance with the law, do hereby depose and state:
1. That I am the same JERSON GUMOBAO who was issued a NBI Clearance by
the National Bureau Investigation Agency and a Voters ID issued by the
Commission on Elections;
2. That in the Certificate of Live Birth, duly authenticated by the National
Statistics Office (NSO), has the following date of birth: October 9, 1980
( A copy of the Certificate of Live Birth is hereto attached as Annex A)
3. That when I filled up my voters registration and my NBI application I
inadvertently and erroneously written the date of my birth as November 9,
1980 instead of October 9, 1980.

That I am executing this Affidavit of Discrepancy to attest to the truth of

the foregoing facts and to explain the discrepancy in my Certificates of Live
Birth, Voters ID and NBI Clearance as regards to my date of birth; Further, I
am declaring that from this date hereon I would be using my true and
correct date of birth October 9, 1980 in all my papers, records and other
documents.

5. That I am executing this affidavit to attest to the truth of the foregoing

statements and for whatever legal purposes it may serve.
