Ralph was late for class in week 1,2 ,9 and 10. Student maintained punctuality throughout rest of the course. Student engaged in class
discussions and asked questions when needed. Student submitted the make up assignments for being late. Student was also referred to
CEC for being late. Ralph submitted the learning plan on punctuality as recommended by CEC. Student is encouraged to continue to
improve on the punctuality standard to be successful for the program.

Student is satisfactory in documentation standard as a student nurse, however student is encouraged to continue to develop on this area.

Ralph is satisfactory in this outcome. Student showed respect for his peers. Student used cell -phone in the class for non-education
purposes for one time in the lab . However, student was able to reflect on his own actions well and did apologized to the writer. No similar
incidents were noticed in rest of the classes.

Ralph is satisfactory in the assessments for cardiovascular, neurological, respiratory, abdominal, and musculoskeletal pathways as
student nurse. Ralph showed good demonstration of pathways in final and mid-term skill testing. Student is encouraged to continue to
practice on the skills learned in the class on ongoing basis and utilize the nursing process as tool. Student is also encouraged to practice
on explaining the steps in any pathway on step by step basis.

To improve and maintain my professionalism and accountability in the future, I need to change my habits to improve my punctuality as attendance in
the clinical setting is very important. Also, I need to keep reviewing my readings to maintain the knowledge and practice to avoid any mistakes from
