K.G. : A student nurse provides the client with the care which is under their scope of their practice and under a supervision of a clinical
teacher. They should meet client needs and should mutually agree on the expected outcomes and they should work collaboratively with
the health care professionalism and develop teaching skills to educate the client. A student nurse should always assume that its their
responsibility to learn in order to develop in their skills and strategies.

difficulty speaking. I have developed over time to even lip read my client who have difficulty speaking. I always update my teacher as well

at one specific time but something else at the other time.

affects and the mechanism of action of the medications and monitor my client according to that. I can answer my client what the
medication is for and how would it help them to relieve those symptoms if they ask me any questions.

noncompliant of suctioning I would encourage him to know the need when to be suctioned in order to have airway clear of secretions.

family wants it for them.

K.G. : I discuss the medication of my client with my clinical teacher and talk about what are they being used for, indications, contradictions

and have talked about it with my teacher.

K.G. : I use my knowledge and skills to prioritize while caring for my clients. Eg. before administering the beta blockers check the patients
BP if its safe to administer the medications because beta blockers lowers the BP

K.G. : I monitor my clients situation and I do my assessments and evaluate them and then prepare my care plan on every shift. I get my
care plan checked by my teacher and get feed back from her.

K.G. : I practice within the legal and ethical requirements. I have been providing my client with the morning care, doing my assessments
and performing vital signs. I have also been safely administering the medications under my teachers supervision. I have practiced doing
suctioning, in and out catheter, giving insulin by checking the sliding scale etc. I am more confident with my skills now than I was before.

K.G. : I gather my information and look for the pros and cons of each evaluation. I havent made any decisions for my clients yet but I can
surely work on it by developing the solutions and setting a time frame to accomplish the goals. But I have shown it through my weekly
care plans about the nursing diagnosis, interventions and outcomes.

K.G. : I have managed to do my morning care, taking vitals and administering medication in a timely and organized manner. I have
developed over time on my nursing interventions and how to prioritize them. Eg: For the clients who are immobile I do re-positioning every
2 hours to prevent pressure ulcers and skin breakdown and do suction my client as ordered and prn and follow clean adn safe procedure. I
have developed on my safe practice, interventions and their expected outcomes.

client I consulted with his physiotherapist about how the exercise will improve his physical impairment

K.G. : When I do my medication research I make sure I retain some information. If in case the patient ask me the question I should be able
to answer them what are the meds for and how will they help them to get better and monitor the patient for side effects of the medication.

reevaluate my clients status, if the expected outcomes are not achieved. If they are not met then I repeat my assessments and evaluate my
clients condition and what can be done in order to provide comfort to them.

K.G. : I prioritize when caring for my client. Eg: If there are any Stat meds need to be given I make sure that the my client have them
immediately or if there are any medication (for seizures) that will be more effective if given at the same time of the day, I make sure that my
client have it during the same time of the day. I suction my client in order to have airway clear of secretions.
