Original & photocopy of birth certificate (& marriage certificate for married females) issued by the NSO. 3.) Current community tax certificate. 4.) Three (3) copies recent passport size colored pleture, white background w/ complete name tag. 5.) Examination fee of nine hundred pesos (P900.00) for baccalaureate degree and six hundred pesos (P600.00) for non-baccalaureate degree. 6.)
