EXTENSION OF LEASE AGREEMENT

This AGREEMENT is entered into by and between:

_____________, of legal age, Filipino, (single / married / widow), and a

resident of_____________, Philippines, (hereinafter referred to as the
"LESSOR");
- and _____________, of legal age, Filipino, (single / married / widow), and a
resident of_____________, Philippines, (hereinafter referred to as the
"LESSEE");

WITNESSETH:
WHEREAS, LESSOR and LESSEE were parties and signatories to
a Contract of Lease executed on _____________ whereby the __Leased
