Religious History:
Lacy Miller is a Christian and was baptized on July 26, 2010. She hasnt attended
church in a long time and feels she needs to find a church she can call a family.
Educational History:
Lacy Miller graduated from Marysville High School in June 2012. Lacy obtained her
Associates of Arts from St. Clair County Community College in May 2014 and is
expected to graduate from Wayne State University with her BSW in May, 2016.
Employment History:
Lacy Miller has had four jobs since she was 15. Lacy started working at Forlakes Golf
Club in August 2010 and quit in August 2011. She worked at ACO Hardware from
June 2011 to August 2013. She worked at Lumberjack from August 2013 to August
2014. She also worked at Gracies Kids Daycare from July 2014 to January 2015.
Strengths:
Lacy believes her strengths are being a hard worker. She has never given up on
school even though sometimes she feels it would be the easiest thing to do. She has
a love for her son that drives her to follow through with school. She is learning how
to stand up for herself which she considers a strength because letting people walk
all over her was a weakness for her for a long time. Lacy believes her family support
is a strength because she knows she will always have her family to lean on in
difficult times.

1. Goal: Lacy Miller will talk with her boyfriend about the feelings she has been
having.
A. Lacy and her boyfriend will each write their top 10 needs from each other
in their relationship

B. They will then switch papers and discuss their needs and ways to help
accomplish the others needs.
C. Lacy will complete this activity before the next meeting.

A. Lacy will go on the internet and look up jobs to do from home.
B. Lacy will pick 5 jobs and fill out an application for each.
C. Lacy will do this and be ready to discuss the jobs she chose at the next

Contract for Intervention Plan

Lacy Miller:
1. Description of Problem:
Lacy Miller is a 21 year old full time college student and mother to her one
year old son. Lacy recently quit her job because she was denied state help with
daycare and could not afford to put her child in daycare any longer. She lives in a
small condo with her boyfriend who is the father of her child. The condo is Lacys
mothers and is letting them stay there as long as they pay the bills. Lacy is
contemplating leaving her boyfriend but feels she needs to stay with him because
she does not want her son to grow up in a broken home and he is also helping with
the bills. She does not want to burden her mother with the financial responsibilities
while she finishes school.
2. Primary Goals:
Lacys primary goals are to talk with her boyfriend about the feelings she has been
having about their relationship and also look for employment she can do from
home.
3. We, the undersigned, agree to the objectives in the following plan:
A. Lacy and her boyfriend will each write their top 10 needs from each other
in their relationship on paper.
B. They will then switch papers and discuss their needs and ways to help
