GOAL SETTING AND PLANNING TOOL

Information from the Self-Assessment tool is used to develop SMART* goals related to student learning and
professional practice. An action plan, along with evidence to be collected, is developed to share with the evaluator.
The educator and evaluator collaborate to refine or revise the goals and plan as needed.

SMART* Goal #1 Student Learning


By the end of the school year, I will assist students to increase metacognition by
facilitating goal-setting activities, and periodically reviewing student progress, in order to
provide opportunities for self-monitoring.
Educator Plan Action Steps:
1. After reviewing their results on the first Embedded Assessment and their score on
CCSD assessments (such as Aimsweb and Study Island), students will develop one
reading and one writing goal.
2. Within the next week, I will conference with each student to further assist them in
developing their goals.
3. During lessons and writing assignments, I will group together students who share a
similar goal and conduct small group instruction with each group to assist to focus
on their goal.
4. I will conference with students on a quarterly basis to review progress made with
their goals and discuss success and struggles they have been experiencing in my
class. Students will have the opportunity to modify and/or change their goals if they
feel they need to focus on something else.
5. At the end of the school year, students will write a short essay reflecting on their
success in accomplishing their goals and elaborating on other concepts they still
need practice in.
Evidence:
Possible sources pf evidence includes goal-setting sheets, videos of one-on-one
conferences, images of students meeting with their goal group, and their reflective essay.
SMART* Goal #2 Professional Practice
By the end of the school year, I will increase multi-cultural awareness within the school by
having students participate in various activities to increase their sense of diversity.
Educator Plan Action Steps:
1. For Hispanic Heritage month, students will be reading and analyzing short stories by
various Latino writers in small groups. They will discuss the themes of identity and
assimilation brought up in the stories.
2. I will continue to bring in a wide array of world literature to read and discuss with
students in class.
3. Spanish club, which I have been conducting for the past two years, will widen their
activities to include not only Latino culture, but other cultures from around the
world. In order to do this, we will set aside one week a month to learn about other
cultures and important figures from around the world.
4. Spanish club will continue participating in family night, providing a multi-cultural
activity for families.
Evidence:
Possible sources of evidence include lesson plans, images of student work, and videos of
Spanish club meetings.
