Training Program Description
Project Training Design
The population to be served by this training program will be limited to the unmet needs
of young racial and ethnic minority male and female between the ages of 20-29 and young
minority individuals living with HIV/AIDS or at high risk for HIV infections. These participants
must be referred to the agency by the Department of Health and Human Services within the State
of Florida.
Article 25 the Universal Declaration of Human Rights
Everyone has the right to a standard of living adequate for the health and well-being of
himself and of his family, including food, clothing, and housing and medical care and necessary
social services, and the right to security in the event of unemployment, sickness, disability,
widowhood, old age or other lack of livelihood in circumstances beyond his control (Universal
Declaration of Human Rights, 2015).
In accordance with Article 25 of the Universal Declaration of Human Rights, the
organization Seeking Hope, Inc. believes that adequate food, housing, social services and
medical care are fundamental human rights, and that the provision of the means to secure these
rights is the obligation of any civilized society. The denial of these rights constitutes the denial of
the inherent dignity and equality of each member of the human family.
The training program goal at Seeking Hope, Inc. is the full restoration of these rights to
people with HIV-related illness, through advocacy with and for people living with AIDS and
HIV, and providing these essentials. Through participation in the Comprehensive Case
Management Training Program, Seeking Hope, Inc. seeks to address the needs of minority men
and women who are homeless, chemically dependent and/or chronically mentally ill and have
been affected by AIDS and HIV, including their partners.

Seeking Hope, Inc. will provide coordinated, timely intensive case management training
services to minority individuals who are encountering difficulty negotiating the complex range of
medical and social services within the entitlement system. While providing intensive case
management services, it seeks to be responsive to the unique needs of every client offering
intensive individually-centered case management training services.
This training program provides client-centered case management services, with particular focus
on:

HIV-infected or at-risk of HIV infection minority men and women;

HIV-infected minority individuals who are chemically dependent and/or chronically


mentally ill;

HIV infected minority men and women who require particularly high level of case
management; and

HIV-infected minority youth ranging from the age of 20 29.


Needs or Gap Analysis
A needs analysis is to be conducted to determine the most effective means of making the

desired change. A survey was conducted based on the feedback received by clients who have
interacted with departmental officers to collect data in order to determine how the
communication gap may be solved through training workshops and weekly meetings. The
positive or negative feedback received by clients will help to identify whether agency staff or
online staff needs more training on improving their communication skills. Also, a survey was
conducted to identify how satisfied or dissatisfied agency clients felt about the availability of
their resources in comparison to other organizations. Based on these two surveys, the results
were that communicational skills need to be improved among agency staff and online training

clients are dissatisfied with their limited ability to enroll into training courses they need for their
education program.
AIDS is fast becoming a primary cause of homelessness across the nation. Even more
than other chronically ill persons, people with AIDS an illnesses related to the Human
Immunodeficiency Virus (HIV) are at a disadvantage in the ever-worsening competition for
affordable housing. HIV-related illness (which is episodic and increasingly debilitating by
nature), often results in homelessness. Fatigue, repeated illness and periodic hospitalization result
in the loss of jobs and the loss of housing. In addition, many persons with HIV lost their housing
as a result of discrimination. Once adequate housing is lost, it is difficult and impossible to
replace.
Seeking Hope, Inc. will be linked with the Florida Department of Health in Orlando,
Florida for their HIV/AIDS Health Education and Prevention program to provide our clients with
HIV and Health educational services. Their training program services include prevention
education through community presentations; facilitate street outreach to at-risk populations; HIV
counseling and testing; provide condoms and educational materials to the community; implement
age-appropriate HIV/AIDS education course plans; and conduct media relationships to ensure
publication of current HIV/AIDS data.

Staffing Plan
Seeking Hope, Inc. Training Program
Work Plan
To effectively administer this training program, the Executive Director and Deputy
Director will hire five new staff positions, namely: a Clinical Social Worker, an Administrative
Assistant/Secretary, a Bookkeeper, a Substance Abuse Counselor, a Mental Health Practitioner,
and a Custodian. The staff will be responsible for the daily activities necessary for training
program implementation. The hiring process will take place within the first thirty days of the
program.
Within 30 days of grant funding contract:
Recruit program staff- Clinical Social Worker, Substance Abuse Counselor, Mental Health
Practitioner, Administrative Assistant/Secretary, Bookkeeper, and Custodian.
Identify the program site.
Develop training curriculum for program staff.
Prepare office space.
Establish separate checking accounts, books and records.
Purchase professional liability insurance.
Within 60 days of contract:
Continue volunteer recruitment.
Obtain additional formal linkages with other community based service providers and healthcare
institutions.
Develop written plan for client intake and case management services pursuant to meetings with
grant funding department.
Solicit in-kind donations for items and clothing for program clients.
Within 90 days of contract:

Develop plan for management system (fiscal, personnel, information and inventory).
Locate support groups and services for program staff.
Continue community involvement such as legal advocacy, fundraising events and resources for
food pantry.

The Executive Director of Training Program is responsible for the supervision of the Program,
which includes client services, supervision of all client support staff, preparation and
management of service contracts, and overseeing training curriculum of all program staff.
Responsibilities:

Establish and implement administrative procedures and policies for Training Program.
Manages client service contracts which include preparation of contract compliance
documentation and billing.
Provides administrative and clinical supervision to all program staff and direct skill
development for each staff member.
Monitors the physical and emotional needs of clients and assesses their capacity to live
independently.
Oversees appropriate levels, type, and quality of care for each client.
Develops supportive service programs for clients.
Evaluates the quality of agencys service delivery.
Represents agency with private and public service providers and AIDS related
organizations.
Delegate additional tasks to support staff as needed.
Implements weekly team meetings.
Shares emergency on call coverage with other client program staff.

Assists the Executive Director in providing comprehensive support services to clients in


the Training Program.
Works together with the Executive Director to provide enhanced services to local
communities by addressing the cultural needs of our population through awareness and
understanding.
Assists with the monitoring of the progress of social service programs.
Monitor organization direct marketing which includes social media, writing/editing blogs,
planning social events, and building relationships with leaders of the local community.
Assists with the implementation of the quality of care for each client.
Monitors charting compliance regulatory agencies- clients with case managers.
Participates in weekly team meetings.
Shares emergency on call coverage.

Qualifications: Bachelors Degree or 2 years of expertise in Human Services. Knowledge of the


needs of persons with AIDS.

Communication Plan
TRAINING AND SUPPPORT OBJECTIVE
Goal # 1 We will, within the first contract year, provide culturally and emotionally sensitive
HIV/AIDS education training to program staff and clients.
Objective 1.1 Within the seven months of the program, staff members will be provided with a

basic introduction to AIDS education (AIDS 101) and another on hygiene- how to care for the
people living with AIDS.
Process 1.1 The Florida Department of Health staff (here after referred to as training staff), will
conduct a two hour workshop in the third month of the program on transmission, immunology,
testing and risk reduction. The following two topics will be presented two months after the
previous training, respectively.
Objective 1.2 Within the last five months of the program, three more trainings will be given in
nutrition and AIDS, treatment modalities and death and dying.
Process 1.2 In the ninth month of the program, the training staff or guest presenter(s) will
conduct sessions in nutrition and AIDS, treatment modalities and death and dying. The last two
topics will be conducted in the eleventh and twelfth month of the program.
Objective 1.3 All events leading up to the effective implementation of each training aspect will
be coordinated by the training coordinator.
Process 1.3 The Executive Director will instruct the training coordinator as to the specifics to be
carried out to effectively coordinate the trainings.
Goal # 2 We will provide support services to program staff, enabling them to develop and
implement the skills necessary to deal with daily stressful events related to their job
responsibilities.
Objective 2.1 The Executive Director will train or schedule training for her public health
educator staff in the self-help modality of support group service provision.
Process 2.1 The Florida Department of Health will facilitate a support group for minority gay
men who are HIV positive or whose lives are impacted by the HIV/AIDS. Groups which are
known to provide support services will be contacted for training literature and/or further training.

Objective 2.1 Support Groups will be held bi-monthly for two hours.
Process 2.1 The training coordinator will consult with staff members to establish convenient
days.
Seeking Hope, Inc. training program has been able to provide enhanced services to our
communities by addressing the cultural needs of our population through awareness and
understanding. We have found that this is a necessary ingredient if we are to effectively nurture
change within our communities. Part of this commitment is demonstrated by reaching out and
helping our community without proper funding. It is imperative that we need funding in order to
better serve our clients. The Executive Director, with her 20 years of experience in social
services, knows what it takes to implement a diversity of programs that will be beneficial to our
communities.
Seeking Hope, Inc. will support harm reduction services and make the necessary referrals
as a method of avoiding disease and encouraging health that meets people where they are
rather than creating conclusions about where these individuals should be in terms of their own
health and way of life. For substance abuse users, harm reduction identifies that many substance
abuse users are either incapable or reluctant to stop, do not want treatment, or are not equipped
for treatment at an assumed time. Seeking Hope, Inc. encourages safer drug use for active
injectors to prevent HIV and hepatitis.
Seeking Hope, Inc. will utilize the Mens Residential and Recovery Program (MRRP) as
a linkage for referrals located in Orlando, Florida for the services that include mental health care,
medication administration, individual & group therapy, case management, support groups,
deterioration inhibition education, emergency management, basic healthiness care, HIV training,
and vocational skills preparation.

Seeking Hope, Inc. will also be linked with the Lakeside Behavioral Healthcare agency
located in Orlando, Florida. This agency will be able to provide our clients with screening for
mental health concerns, substance abuse conditions and supplementary addictions. They also
provide innovative, resourceful and compassionate in their treatment and recovery approaches as
well as cost effective behavioral health care services.
During the intake process, we will assess the potential clients needs to determine
whether or not the client is an eligible candidate for our training program.
This will be done in conjunction with the referral agency case management team as well
as our team. Our clients needs will be assessed on a monthly basis to ensure proper service and
accurate reporting on monthly reports to our granted funding source(s).
Program Evaluation
Seeking Hope, Inc. - Training Program
REPORTS, DATA COLLECTION AND EVALUATION
As per funded grant requirements, reports will be generated at the end of each project
month. These progress reports will include an assessment of each case from the point it was
called in to us, with information such as client progression, condition of living environment, and
health status. This will also be accompanied by information gathered from the most recent visit
and based on the findings of each assessment conducted which would include any problems that
need to be addressed.
Samples of forms with possible format will be delivered to the grant funding provider.
Program progress will be evaluated through monthly staff meetings with problems being
addressed by the Executive Director and/or an executive staff member when necessary. The
report containing the data collected will be sent for review by the funding agency. A monthly

report will include accountability of expenditure, individual client progression, and efficiency of
the training program.
The Florida Department of Health will provide the training and support services required
for the training Program. The training programs present Executive Director will be responsible
for providing additional training and support to the staff and clients. She will seek public health
educators to bring into the facility to provide services to the organization. The Executive Director
will develop linkages with different supporting organizations in order to recruit professional
volunteers who are willing to donate their time and services such as nurses, doctors, lawyers,
educators, etc.
TRAINING EVALUATION
1. A Knowledge Attitude and Behavior (KAB) study will be conducted as a part of the first days
training. This study will be repeated at the end of the fiscal year to the same group, a 40% change
in KAB in a positive direction will determine effectiveness of the training and support process.
2. At the end of each In-Service, a satisfaction questionnaire will be distributed to each
participant for presentation evaluation.
3. By the third In-Service, workers will be asked to evaluate the program to date and asked to
make suggestions for improvement.
4. Trainer will evaluate each presentation as to the reason (need) for the presentation, the format
used to address it and how effective it (format) was.
5. All these forms will be maintained in a monthly log.
6. Executive Director will meet the staff monthly to assess program progress and discuss any
suggestions they may have or which were recommended to them by the trainees.
