Scope of this Product Comparison
This report covers infant incubators used for intensive, intermediate, and primary care. It also
covers transport infant incubators and one unit
that can be used as either an incubator or a
radiant warmer. Dedicated radiant warmers are
covered in the report titled WARMERS, RADIANT,

conduction (heat loss to cooler surfaces in direct contact with the infant), convection (heat loss to air moving past the infant), radiation (heat loss to cooler
objects not in direct contact with the infant), and water
evaporation (heat loss from the infants lungs and skin
surface). Whereas term neonates naturally regulate

Purpose
An infant incubator provides a closed, controlled
environment that warms an infant by circulating
heated air over the skin. The heat is then absorbed into
the body by tissue conduction and blood convection.
Ideally, both the skin and core temperatures should be
maintained with only minor variations. Incubators
feature better temperature control than radiant warmers (overhead heating units), which can overheat or
underheat the infant. However, an infant in an incubator is less accessible to the caregiver.
At birth, an infants core and skin temperatures
tend to drop significantly because of heat loss from

their body temperature to some extent, premature
infants have thinner skin, which allows surface blood
vessels to more readily lose heat to the environment; a
large ratio of surface area to volume, resulting in
greater heat losses from radiation and convection; and
almost no subcutaneous fat to either metabolize into
heat or act as an insulator. Prolonged cold stress in
neonates can cause oxygen deprivation, hypoglycemia,
metabolic acidosis, and rapid depletion of glycogen
stores; therefore, energy conservation provided by
thermal support is critical.

Principles of operation
The neonate lies on a mattress in the infant compartment, which is enclosed by a clear plastic hood.
Most incubators have hand access ports with doors
that permit the infant to be handled while limiting the
introduction of cool room air. The clinician can raise or
remove the plastic hood or open a panel to gain greater
access to the infant. Some units feature an air curtain
that causes warm air to sweep past the opening.
Most incubators warm the infant by a forced or
natural flow of heated air. At least one unit supplements air convection by actively warming the incubator walls to reduce radiant heat loss. Another unit uses
a mattress of warm water, rather than a convective
airflow, to warm the infant.
Heating and humidification systems are located beneath the infant compartment. A fan or natural flow
circulates air past the heater and the temperature
measuring device, over a water reservoir used to humidify the air (if desired), and up into the infant
compartment. Most incubators are equipped with proportional heating controls that provide electrical
power to the heating coil in response to the difference
between the actual temperature and the desired temperature. Most units have two modes of operation:
air-temperature control and skin-temperature control.
With the air-temperature (manual) control, the operator sets the temperature of the air in the incubator;
changes in infant body temperature are usually measured periodically with a thermometer, and adjustments in air temperature are made accordingly. In the
skin-temperature control mode, also called the servo
(automatic) mode, a sensor is taped to the infants skin,
and the heater responds to changes in the sensor to
keep the skin temperature at the preset level.
Most units allow the user to vary relative humidity
from either a built-in reservoir or an outside source
(e.g., a humidifier that attaches to one of the inlet
ports). Although increasing the relative humidity in an
incubator can reduce evaporative heat loss, many clinicians avoid supplemental humidification because of

concern that infectious organisms may proliferate in
the water reservoir.
Many incubators have one or two oxygen inlet ports
and can be equipped with optional oxygen controllers.
The incubators can also provide support and protection for oxygen cylinders when oxygen must be delivered to the infant in the incubator.
Because the room temperature of the nursery is
nearly always lower than that inside the incubator,
radiant heat loss through the incubator walls accounts
for as much as half of the infants total heat loss. In
some nurseries, a plastic heat shield is placed over the
infant inside the incubator to minimize radiant heat
loss. In addition, some incubators have double walls
separated by an air space to prevent excessive heat
loss. However, in a study comparing heat loss from
servo-regulated single- and double-walled incubators,
Bell and Rios (1983) reported that, although the double-walled incubator decreased radiant heat loss, it
increased convective heat loss; total heat loss and
metabolic heat production were the same as in singlewalled incubators.
Incubator/radiant warming units
One manufacturers device combines the thermal
capabilities of an infant incubator with the access
advantages of an infant radiant warming unit. This
hybrid unit provides a centralized control and monitoring system for both the incubator and the warming
unit; it can be converted automatically from a closed
incubator to an open bed radiant warming unit and
vice versa with little effort in a few seconds. This device
eliminates the need to transfer an infant between an
incubator and a radiant warming unit.

Transport incubators
During transport within a hospital or to another
facility, infants require thermal support. Transport
incubators can be operated on a variety of power
sources found in hospitals or transport vehicles (e.g.,
120 VAC, 12 VDC, 24 VDC). Most units also carry their
own backup power supplies (e.g., 12 VDC rechargeable
nickel-cadmium [Ni-Cd], gel cell, or lead-acid batteries) that power the unit for short periods in the absence
or failure of other power sources. In addition to having
a reliable portable power source, transport incubators
also need to meet strength requirements for air and
ground travel, provide isolation from external noise
and vibration, and have limited electromagnetic emissions to allow for aircraft use. Transport incubators are
typically smaller and lighter than mobile or stationary
incubators, facilitating easy maneuvering in and out
of emergency vehicles.

Reported problems
Deaths and injuries to neonates in incubators have
been linked to thermostat failure that caused incubator overheating and infant hyperthermia and to malfunctions or design defects that produced fires and
electric shock hazards.
If a surface temperature sensor does not make adequate thermal contact with the infants skin, incorrect
low temperature readings may result, and the incubator could overheat the infant in response to the false
readings. However, using a strongly adherent tape to
fasten the sensor can produce skin irritation or damage. Periodic checks of sensor placement are strongly
recommended.
Inadequate control over the amount of oxygen delivered in an incubator can cause hyperoxia or hypoxia
(excessive or inadequate oxygen, respectively, in body
tissues). Excessively high concentrations of oxygen can
contribute to retrolental fibroplasia (i.e., formation of
fibrous tissue behind the lens, leading to detachment
of the retina and arrested growth of the eye) in premature infants.
Incubators can produce excessive internal noise levels that might adversely affect infant hearing. Slamming a porthole, using a nebulizer, activating an
alarm, and tapping on the incubator hood further
increase the noise level to the infant; levels as high as
90 to 100 dB have been reported. The 1991 Association
for the Advancement of Medical Instrumentation
standard for incubators specifies that the maximum
noise level inside the infant compartment should not
exceed 60 dB during normal use.

In a case reported to ECRI, an infant was able to


push open the rear isolette door of a mobile infant
incubator due to improper door gasket placement. The
mattress in the isolette placed the infant at access-door
level, and the infant fell out of the incubator to the
floor.
Some older incubators still in use may have hood
thermometers or high-temperature thermostats (or
both) that are filled with mercury. Mercury contamination is possible if the thermometer or thermostat
breaks, which is most likely during routine cleaning or
maintenance. Several investigators have expressed
concern about possible adverse effects of an infants
exposure to mercury vapor. It is not known to what
extent safe limits for chronic adult exposure apply to
sick or premature infants who remain in incubators for
extended periods of time. Some investigators recommend replacing all mercury-containing components in
infant incubators, and most hospitals have followed
this course of action.
ECRI has published an infant incubator checklist

and Preventive Maintenance System below) that can
help detect many potential problems with incubator
equipment before they occur.

Purchase considerations
Infant incubators typically cost between $4,000 and
$14,000, depending on accessories purchased. Higherpriced incubators may include such features as a
mechanism that minimizes the temperature drop
when the front-access panel is opened, a rail system to
mount accessories, a humidification system, and an
electrical height adjustment capability.
Hospitals can purchase service contracts or service
on a time-and-materials basis from the supplier. Service may also be available from a third-party organization. The decision to purchase a service contract should
be carefully considered. Purchasing a service contract
ensures that preventive maintenance will be performed at regular intervals, eliminating the possibility
of unexpected maintenance costs. Also, many suppliers do not extend system performance and uptime
guarantees beyond the length of the warranty unless
the system is covered by a service contract.
ECRI recommends that, to maximize bargaining
leverage, hospitals negotiate pricing for service contracts before the system is purchased.

Stage of development
Manufacturers continue to improve the design of
incubators, focusing on thermal protection. Many

infant incubators may be upgraded to an intensive care
workstation with several accessories.

A4070 FDA has designated Class II Recall Nos. Z234/237-0 certain Hill-Rom Air-Shields K-1 infant
transport incubators, which have a faulty design of the
hand ventilator that does not indicate the true pressure inside the incubator. The manufacturer initiated
a field correction by site visits beginning October 15,
1999, and by letters dated March 3 and April 6, 2000.
Verify that you have received the letters, and identify
and isolate any affected product in your inventory.

Supplier information

A4363 O 2 concentration variations can occur in


Ohmeda Medical Giraffe OmniBed infant incubator/
radiant warming units, resulting in too much or too
little O2 inside the infant compartment when the units
are operated in incubator mode. The variations occur
as a result of changes in the fan speed when the units
are operated in incubator mode and free-flow O2 is
administered. Changes in the fan speed may occur at
any time in response to the temperature of the bed
heater or as the result of intentional user activation.
The manufacturer states that it has not received any
reports of injuries arising from O2 concentration variations. Injury is possible only if (1) free-flow O2 is being
administered, (2) no servo-controlled O2 delivery system is being used, and (3) the O2 saturation of the
