With reference to our recent observation, it has brought to our attention that
despite of being given repeated verbal warnings and a warning letter dated
September 24, 2015 with regards to your not coming to scheduled work on time,
you still incurred consistent tardiness. This notice is being given to you for your poor
performance over the past 4 weeks due to your consistent tardiness.

Please note that coming late to work affects not only your personal work but also
the entire process where you are involved in.
Hope the above is understood, accepted and put into practice by you
immediately. Non-compliance of the same will result in dismissal of your
employment for San Carlos Company.
