yourname),EmployeeID,oraNotarizedAffidavit(inEnglish)withPhoto,Signature,DateofBirthand
ResidentialAddress.
d. Photocopyoftheidentificationproofthatyouarecarrying.Youneedtocarryoriginalidaswellaswiththe
photocopy.
e. NecessarystationarywhichmayincludePen,pencil,eraser.
2. RoughSheetswillbeprovidedtoyouatthetestcenter.
3. CollegeIDisonlyacceptablewithavalidATM/Credit/DebitCardwiththesamename.
4. IfindoubtaboutthevalidityofyourID,pleasecarryaNotarizedAffidavit(inEnglish)withphoto,signature,dateof
birth,andresidentialaddress.
5. Youneedtoreporttheexaminationvenueatleast15minutesbeforethestarttimeasmentionedintheadmitcard.
CandidatesreachingafterthereportingtimewillNOTbeallowedtotakethetest.
6. YoushouldkeepyourmobilephonesswitchedoffwhiletakingAMCAT.
7. Anycheatingmaterial/instancefoundduringthetestwillleadtoimmediatecancellationofAMCATwithoutany
