What Happened?
Whilst Pressure Testing HCR & Remote
Choke Valves, via a 1502 test flange with
incorrect bolts fitted, flange released with
a bang (contained on remaining bolts). On
hearing the sound the I.P. stumbled &
struck his head, causing a small 1 cm
laceration behind his right ear. All barriers
& permits were in place.

What Caused This?


1. Incorrect bolts used on the flange as per
API SPEC 6Aresulted in short bolted
connection that could not withstand
test pressure.
2. Supervisors accepted an unsafe
condition.
3. Crew members failed to use the Stop
Work Authority.

What Can I Do?
