FIELD TRIPS
MEDICATION ADMINISTRATION
-For Principals Designee

TUSD POLICY REGULATION JLCD-R


Field Trips (Not Interscholastic)
Prescription medications must be in a
pharmacy container labeled with
students name, date, medication,
dose, time to be taken.
Day Trips
On day trips medication may be
administered by a principals designee
(certified district employee).
Parents must supply an extra
prescription container to send on the
field trip. If an extra prescription
bottle is not provided health office
staff will send the school container,
with its entire contents, unless other
arrangements are made.

Overnight Trips
On extended trips medication may be
administered by a principals designee
(certified district employee). The parent must
complete the Overnight Travel Emergency &
Medication Form (HEA 2108).
The parent must give the medication directly
to the principals designee (certified district
employee) in charge of the trip and perform
any necessary teaching in regards to giving
the medication and/or specific healthcare
needs.
The medication must be kept in possession of
or under control of the principals designee
(certified district employee).
Exceptions:
Emergency medications allowed to be carried by
the student per state law.
Birth Control and over-the-counter medication
may be carried by the student with a note from
the parent directing the use and possession of
the medication (middle & high school).

5 RIGHTS OF MEDICATION
ADMINISTRATION
Right Person:

If the name on the container is not the name the student gives you when
asked What is your name?, DO NOT GIVE THE MEDICATION Notify the nurse.

Right Medication:

Read the Rx label after you pull the medication from your secure
storage. It should match the medication listed on the field trip form.

Right Dose:

Read the Rx label carefully. You may need to give one pill, two pills, half a pill,

etc

Right Time:

A medication may be given hour on either side of the prescribed time.


Example, give at 12:00 daily may be given between 11:30 and 12:30.

Right Route:

Read the Rx label carefully. Common errors are eye drops in ears or ear drops in
eyes. Some students may have special instructions that are not on the prescription label. For
example, crush pill and mix with applesauce. The nurse or parent will inform you of any
special instructions.

Field Trip Medication Log or Student Overnight Travel Medication Log.

Right to refuse:

If student refuses medication dont force student to take it. Document


refused and notify school nurse for day field trip or parent for overnight field trip.

medication to the health office.

SELF ADMINISTRATION of RESCUE


& EMERGENCY MEDICATIONS
Prescription medications which have been identified
by state law as being approved for self administration
may be carried and self administered by the student.
The following prescriptions for self administration are presently
allowed by State law.
Inhaler devices
Auto injectable epinephrine
Diabetes Medication (Insulin & Glucagon)

The medications should have a prescription label on them.


The parent will have consulted with the school nurse and health
services to verify expectation regarding the use of the medication
and will have a written emergency action plan on file.
The school nurse will review the students emergency action plan with
the principals designee.

The school nurse may train the principals designee in the


administration of certain emergency medications if applicable.

MEDICATION ABSOLUTES
Store medication in secure place. Do not
let students carry their own medication
Exceptions:
Emergency medications allowed by law
Birth control and over-the-counter medications allowed on overnight trips
with parent note (middle school & high school only)

Dont accept medication from a parent


unless it is in a prescription bottle or
appropriate container.
Never accept medication in a baggie, tinfoil, etc

Never allow students to dispense their


own medication(s).

PROCEDURE FOR ADMINISTERING


MEDICATION

Have the field trip medication log(s) with you when you give the medication.
Ask the student to state his/her full name.
Obtain the medication from secure storage, read the name on the labeled
container and compare with the medication log(s).
Repeat the students name out loud.
Compare the 5 rights.
Gently shake dose into a medication cup.
If the student takes two medications use two medication cups.
Set the medication cup on a counter or other available stable surface.
Have the student drink water with the medication and observe the student
while swallowing the medication. Dont assume the medication was taken
because you gave it. You must observe the student swallow the medication.
Record exact time the medication was given on the field trip medication log.
Place medication container back into secure storage.

Return the empty container or the container with extra doses to the health office when you
return from a day trip.
Return the empty container or the container with extra doses to the parent when you return from
an overnight trip.

If more than one student
needs medication at the

After being instructed regarding
medications and procedures the
principals designee will sign the form.
The principals designee will document
the amount, route and time the
medication was given at the bottom of
the form.

The nurse will give the principals
designee an Overnight Travel
Medication Log for each medication
to be given.

Students in middle school and high


school can carry and selfadminister over-the-counter
medication with a written note from
the parent.

The principals designee will sign that the


parents gave instructions and that the
principals designee accepts responsibility for
giving the medication.

The principals designee will log all


medications given.
The principals designee will return the
medication container to the parent at the
end of the trip and both will sign for the
amount returned.
The principals designee will return the log
to the health office upon returning from the
trip.

MEDICATION ERRORS
Incident reports will be filled out for any
medication errors:
If any of the five Rights are not followed
Wrong student, wrong medication, wrong dose, wrong time, or
wrong route

If a medication is not given


If a medication was given but not documented
If a serious medication error/incident occurs
911/Child is sent to the hospital
Doctor was consulted
Adverse reaction

Notify the school nurse and your immediate school


administrator of a medication error
The nurse will instruct and support the process

SUMMARY OF INCIDENT REPORT


All Incident Reports must be submitted to
the Director of Health Services within
twenty-four hours of the incident.
Notification is made to the:
Nurse
Parents
Principal
Possibly Risk Management
If warranted, the physician or poison
control may be called

OTHER
If you have any questions dont
hesitate to ask your school nurse
before the trip.
