It was three in the afternoon and the member was in his bathrobe
looking like he had not shaven in months but he was agreeable
to the visit, so we began.
When it came time for the medication review, he just directed me
to a cabinet next to the sink where I found months worth of
unopened prescription bottles. The medications were for diabetes,
heart disease, atrial fibrillation, COPD and an anticoagulant.
The member said, I havent taken my medicines in months, I
want to die. He said that he did not have a plan but, If God took
me today, I would be very happy.
I contacted the members primary care provider and shared
everything from the visit. The doctor said he could see the
member. I contacted the Area Agency on Aging and assisted them
with the intake information.
I checked to make sure all of the members utilities and telephone
were working. With permission from the member, I then contacted
his brother, who was his emergency contact. He thanked me for
the call and assured me he would check on his brother as soon as
he was able.
In a follow-up call, the brother said he took the member to the
doctor and the member was receiving a full medical workup.
