Native Village of Eyak is soliciting Requests for Proposals for a

Patient Centered Medical Home, Meaningful Use compliance contractor for our
Ilanka Community Health Center
Work is anticipated to be completed mainly off-site, with some on-site work to meet with providers and
staff, and attend quality initiative and quality assurance meetings. The successful proposal is anticipated to
result in a one year contract, renewable for an additional year with mutual consent. Time spent on
deliverables is anticipated to be spread out throughout the year and work is expected to take a couple days
per week.

Contract Deliverables
Work with Ilanka Community Health Center infrastructure to achieve:
Continuing compliance with Meaningful Use stage II according to 42 CFR, professionals and
ambulatory clinics.
Compliance with Meaningful Use Stage III according to proposed rules issued from CMS, and with
stage III using final rules as soon as they become available.
Ensure Ilanka Community Health Centers continuing certifications with Meaningful Use.
Continuing compliance with level II accreditation Patient Centered Medical Home through the
National Committee of Quality Assurance.
Level III accreditation for Patient Centered Medical Home through the National Committee of
Quality Assurance.
Educate Ilanka providers on effective use of electronic health records software to obtain above
deliverables.
Work with EHR software providers to ensure updates, patches and interfaces have the capability to
provide information necessary to obtain above deliverables.
Work with Ilanka information technology staff to develop and implement appropriate protocols,
policy and procedure as necessary to obtain above deliverables.

received in the NVE main office by 5 PM on 6/8/2015. NVE reserves the right to reject any and all bids for
any reason.
