Question 4
According to Section 30 of Occupational Safety and Health Act (OSHA) 1994, any workplace
which has 40 or more workers are required to establish a safety and health committee. The
committee acts as a platform for both the employer and employees to discuss on occupational
safety and health (OSH) matters at the workplace.

In recent years, Hazard Identification, Risk Assessment and Risk Control (HIRARC) has
become fundamental to the practice of planning, management and the operation of a business
With HIRARC, one will be able to identify hazard, analyse and assess its associated risk and
then applies the suitable control measures. HIRARC is a preventive measures in managing
OSH at the workplace.

Question 1
Each year, work related accidents at workplace are increasing and over the years, several
theories of accident causation have evolved that attempt to explain why accidents occur.
Models based on these theories are used to predict and prevent accidents.

events that lead to an accident.

Accidents are preventable. In order for us to prevent these accidents; we need to understand
the common causes of accidents. Based on Hartshorn, there are various categories of common
causes of accidents.

It is common nowadays that most people at work interact with various types of machines;
therefore they are exposed to potential incidents and accidents while handling these machines.
a. Describe THREE (3) common mechanical injuries associated with machines.

b. Describe THREE (3) point-of-operation guards that can be used to safeguards workers
from exposure to moving parts.

Question 4
More than 16 percent of all disabling work-related injuries are the result of falls. Safety
professionals recognise two categories of falls:
1. Elevated falls, which ocour less often but are more severe than same-level falls; and
2. Same-level falls occur more frequently but are generally less severe than elevated falls.
