(such as iPad) Other wireless device (such as iPod Touch)
Describe the instructional activities that will occur prior to the SRS
Activity and how you will introduce the SRS Activity: I will use this particular
SRS Activity as a review/EQ, or as a Content Knowledge Assessment. I will instruct
them on how to use the i-clickers or phone tablets prior to doing this activity.
Describe the purpose of the SRS activity (Check all that apply)

Additional explanation of purpose (optional):
Type of session: Teacher-Paced Student-Paced Im not going to rush my students through
the quizlet, but Im also not going to let it drag on and last a long time when it should not take that long.
Identification of students:
Students will log-in so their individual scores will be available to you after the session
SRS Activity will be anonymous
Briefly describe what will happen during the SRS Activity (For example:
What will students do? What will teacher do? What materials are needed? How
long will the SRS activity take?):
Students will be shown a word, and they have to identify the math symbol related to it. The teacher will
flip through each card on quizlet and monitor the time. All that is needed is a tablet or i-clicker. This
activity should take no longer than 10 minutes.

Types of Questions/Prompts (Check all that Apply):

Multiple-choice Multiple Select True/False Yes/No Short Open-ended response or fillin-blank Longer open-ended response
Provide samples of questions/prompts to be given to students:
A math term for a symbol will be shown on the board. Students will either have to
use multiple choice and choose the correct symbol, or draw in the symbol on their
own and submit. This activity could also be made into true/false and yes/no
format.
Right/Wrong Answers: Will there be right/wrong answers to these questions? :
Yes No Mixed (Some will have correct answers, others will not.) The answers are
either right or wrong. There is not a right and wrong answer to each question.
Immediate Corrective Feedback:
Will you pre-select correct answers to some or all of the questions and display the
correct response to the class after the SRS activity? Yes No
Why or why not? I am going to show the correct answers after each question because this activity is
made in flip card format. Students will know right then whether they got an answer right or wrong but I
am also going to go check.
Use of Data: What data will be collected as a result of this activity. How will it be
used and by whom? (For example: Will information collected from this activity be
used to award a grade? Will the individual information collected be shared with
students and/or parents to help them monitor individual progress? Will you
discuss the aggregate, anonymous data with the whole class to help them learn?
Will you use data to differentiate instruction for students? If so, describe how. ) I
will use this to determine if we need to review math terms and their symbols. IF a student missed most
or all of the answers then I would give extra help to that student and speak to his/her parents to see
about reviewing at home as well.
Describe what will occur after the SRS activity: Once the activity is done, students
will hand in their i-clickers or tablets and I will review the data and see if we need to review any math
terms and their symbols.
Describe your personal learning goal for this activity. (For example: What
are you trying that you have not tried before? What do you hope to learn from
this activity? How do you hope it will help students learn? You must design
something that will help you learn something new! Honor System!) I hope
students will remember all their math terms and symbols because they will use
them for the rest of time whether they think they will or not.
