Cell Phone Warning

By signing below, I acknowledge I have used my cell phone inappropriately during class, and
did not put it away after my teacher had asked me to do so.
If I violate the class cell phone policy again, I understand that the following consequences will
apply.
1. My cell phone will be confiscated, and turned in to my Academy Dean. I will have to pay
$15 for my cell phone to be returned to me.
2. If I refuse to give my cell phone to the teacher, then the teacher has the option of writing
a discipline referral for my failure to comply with a reasonable request.
