PROGRAM AND SERVICES

Improve development, preventive program


for risky behavior, social participation
promotion.
Mentor Program. Pairing children with
mentors/mentees depending on grade levels,
promoting development through
Occupational Therapy based activities
including but not limited to: gross motor,
fine motor, sensory integration, and
academic activities.

WHO IS THIS FOR?


1st

Through 8th Grade Students

MENTORING GROUPS:

4 mentoring groups will include individual


pairings of:
1st

& 3rd graders


2nd & 4th graders
5th & 7th graders
6th & 8th graders

WHEN WILL THIS PROGRAM


TAKE PLACE?

Program will take place Tuesday and


Thursday weekly after school from
3:00pm-4:30pm.

GOALS

Students will increase their social


participation by engaging in therapeutic
activities with peers.

Students will increase performance in school


and academic productivity outside of class.

Students will meet or exceed their


developmental milestones according to age.

PROGRAM SCHEDULE:
3:00pm-3:10pm Snack
3:10pm-4:10pm Paired groups
will rotate between stations of
obstacle course (gross motor),
fine motor games/crafts,
sensory fun, and go noodle
music group
4:10pm-4:30pm Homework and
Mentor Discussion

ACTIVITIES:

Snack. Eating snack with mentor while visiting.


some days will have making simple snack
together (fine motor, teamwork, attention to
task)
Obstacle Course. Will complete with mentor
partner. Student will complete course as set up
and sometimes set up their own course (gross
motor, teamwork, provide proprioceptive,
vestibular, tactile, & visual input)
Fine Motor Games/Crafts. Multiple fine motor
activities will be provided. Students may choose
which activity to complete with their partner.
Activities will include crafts, board games, etc.

ACTIVITIES CONTINUED:

Sensory Fun small area with multiple sensory


input items: bean/rice box, play dough, water
table, ball pit, bean bag chairs, mats, etc.
Go Noodle music group: children will watch
their choice of kid-friendly music videos while
dancing and imitating movements. Therapeutic
listening activities. Playing instruments.
Homework/Mentor Talk: children will have 20
minutes to work on homework with mentor, ask
mentor questions about upcoming grades, discuss
school work, etc.

FINANCIAL CONSIDERATIONS

Grant money provided from school will cover


initial funding needs, materials needed, and
staffing.

STAFF NEEDED

One Registered Occupational Therapist


Two Certified Occupational Therapy
Assistants
Multiple Volunteers from High School Student
Population (Credit for Certain Class)

PROGRAM EVALUATION PLAN

OTRs and COTAs will continuously document


the students progress
Submit a new survey to parents once a
month
Discuss students home behavior and
homework improvements with parents
Staff meetings and discussion every two
weeks
