Part A: Bio-Psycho-Social Assessment
Introduction:
Stephanie Jones (pseudonym) is a 39-year-old American female who is currently stressed out
with her work and financial situation. She is studying to become a pastry chef and got a job at a
cookie shop in the fall and worked there for a few months. In the beginning of February she quit
this job because she simply was not making enough money to survive. The cookie shop lost a lot
of business after the holiday season, leaving Stephanie with less and less hours. With this, higher
bills due to the cold weather, and a roommate who stopped paying her part of the rent and
eventually moved out, this put Stephanie in a tough financial situation. She just started a new job
painting for a friends business, a job that she has never done before but one that will be able to
provide a better income. All of these life changes and difficult situations have left Stephanie
feeling quite stressed out, though she is hopeful that her new job will work out and help her get
back on her feet.
Family of Origin History:
Stephanies parents were married and both 26 at the time of her birth. Both parents had only
completed their education up to high school level. She also has a brother, Greg, who is 35 and is
a healthy single man working as a district manager of a 7-Eleven. Greg has received his
Associates degree and is now working on getting his Bachelors degree. Both Stephanie and
Gregs births were planned. The parents are now divorced and this is due to the father cheating
on the mother when Stephanie was still quite young. Both parents are now 65 years old.
Stephanie has not kept in contact with her father but is assuming he is healthy because she
believes that she would be notified if that were not the case. Stephanies mother is not very
healthy. 17 years ago she was diagnosed with bipolar disorder, she had a stroke 10 years ago,
which left her with paralysis on the left side of her body. She also has diabetes, Parkinsons
disease, and is now in a nursing home. As a child growing up, the atmosphere of the home was
that the mother raised both kids while also working and trying to go to school. Holidays were
often celebrated with the mothers family, including Stephanies Aunts and Grandmother. They
often celebrated holidays like Christmas or Easter at her Grandmothers house. Stephanie never
really had a great loss or death in the family that she had to deal with, although her brother did
lose a few close friends and according to Stephanie he had handled it the best that he could.
Stephanie never experienced any physical or sexual abuse as a child, although a boyfriend did
physically abuse her about six years ago. As a child the only abuse she feels she received was
mental and emotional abuse from her mother.
Developmental History:
The length of pregnancy at Stephanies birth was 9 months. She weighed 5lbs 6 oz. with no
issues present at birth. Her mother had a vaginal delivery and Stephanie was breast fed with no
feeding issues, and then was weaned at about a month old. When asked the age of any
developmental milestones, such as the age when she first walked, talked, or was potty trained,

Stephanie stated that she does not know or remember. She was never told she had any
developmental delays or was advanced developmentally.
Health History:
At birth, Stephanie was healthy and the only childhood illness she went through was the chicken
pox. She had no surgeries, chronic illnesses, accidents, or broken bones as a child. She was 15
years old when she had her first menstrual period. As an adult, Stephanie had bronchitis once and
had one surgery due to a broken wrist. She has had no real weight issues in her lifetime. Health
issues that are common in her family are diabetes and high blood pressure, though Stephanie
herself does not presently struggle with these issues. Currently the only mediation Stephanie is
taking is birth control in the pill form. She is allergic to Neosporin and similar creams and
products.
Relationship/ Sexual History:
In regards to childhood relationships, she still remains friends with one girlfriend that she had
when she was 14. Currently she has a lot of friendships and maintains a healthy social life. On
average her current friendships have lasted from about 5-10 years. Stephanie is straight and had
begun dating at the age of 16. Her first sexual experience was when she was 17 and she has a
good and healthy ability to enjoy sexuality and have orgasms. She is not now in any sexual
relationships, but is still on birth control in the pill form. Stephanie has had no sexually
transmitted diseases and still does not.
Family of Creation History:
Stephanie was married once and had one other long-term committed relationship that lasted for
four years. She was married at the age of 22 and her husband was 26 at the time. The reason for
the marriage was that she was young and in love. They had no children together. The man she
married had high school education and worked at a plant that built machines that were used to
then build cars. The marriage lasted four years and they ended up getting divorced when
Stephanie was 26. The reason for the divorce was that he started working midnights, which then
caused them to start to fight frequently. The other long-term relationship that she was in ended
because he was what she called a drunken idiot. Stephanie accidentally became pregnant two
times in her life but had no children. She had one abortion and one miscarriage. She never
suffered any abuse in her marriage or with her other long-term partner but did suffer physical
abuse with a different boyfriend.
Substance Use/ Abuse History:
Substance abuse was and is prevalent in Stephanies life with use of marijuana and alcohol. Her
first use of marijuana was when she was 15. She stopped using it last June. It was an easy change
for her to make and she has had no desire to smoke marijuana again. Stephanie was 14 when she
started drinking and it is something that has been more of a struggle for her. She recently became
sober and stayed sober for 13 months, and then started to drink frequently again. She is now
trying to get sober again and has been in AA for three weeks doing her best to get back where
she wants to be. The only treatment she has received for substance abuse has been through AA.
When asked why she thinks the treatment has failed she stated that she is unsure but is now in
counseling to figure it out and get through her issues.

Mental Health History:


Overall the client feels that her mental health is positive. She has had some issues that she has
had to work through and still is, but she keeps a positive attitude and tries to get through it the
best that she can. She has received counseling for physical abuse, substance abuse, and
depression.
Financial History:
While growing up, Stephanie and her family struggled a bit financially. She considered them to
be poor and they lived in low-income townhouses. In regards to her financially situation
currently, she feels that she has to struggle to get by right now and that is one of the big stressors
in her life.
Legal History:
In 2001 and 2003 Stephanie was arrested for drunk driving. In 2007 she was arrested for driving
on a suspended license three times. In 2012 she was arrested for punching an ambulance worker
in the face. She is not currently on probation or parole, all of her charges are paid off, she has no
warrants out, and she is working on getting her license again.
