address at 209-211 Mc Arthur Highway, Karuhatan, Valenzuela City, do hereby name,
constitute, and appoint MR. WILLIE A. YAGO, likewise a person of legal age, single,
Filipino, and with residence address at No. 61 Lopez Jaena Street, Brgy. Doa Aurora,
Quezon City to be my true and lawful attorney-in-fact, for me and in my name, place and
stead to do and perform the following:

To process with the Philippine Health Insurance Corporation (Philhealth) my
application for Philhealth Pensioner/Retirement Identification Card.
2.
To process with the Social Security System (SSS) my application for
Pensioner/Retirement benefits.

To sign, execute and deliver any and all document necessary for the
accomplishment of the above given authority.
HEREBY GIVING AND GRANTING unto my attorney-in-fact full power and
authority whatsoever requisite or necessary or proper to be done in or about the premises,
as fully for all intents and purposes as I might or could lawfully do if personally present, and
hereby ratifying and confirming all that my attorney-in-fact shall do or cause to be done by
