1. Name of Organization: GLIDE: Guiding Life to Independence through Development
and Education
2. Mission Statement: We will seek to work with the foster care system in our region in
preparing foster youth for transitioning from foster care toward independence. Through
compassionate care we will strive to provide the education necessary, to teach life skills
to young adults leaving the foster care system.
3. Philosophy: To model the 7 core values in all of our communications with our clients
and each other, so that by our example, it may become their work. We will provide
services to all regardless of race, sex, gender, disability, religion, or sexual orientation.
a. Core Values:
i. Mutual Respect: By modeling respect to our participants and to each other,
we can teach the value of mutual respect.
ii. Nonmaleficence: In modeling the concept of not harming others with our
words or our actions, we can teach our participants how to appropriately
handle stressful situations without resorting to violence.
iii. Beneficence: Through continually encouraging the duty of all support staff
and participants alike we will promote the good for others and their benefit
from all aspects of the program.
iv. Empathy: We will teach the value of empathy by providing sensitive and
appropriate care.
v. Integrity: To hold support staff and participants to the utmost respect and
create and healthy and safe environment for all through open
communication and honesty.
vi. Fidelity: By upholding personal and professional contracts between
support staff and participants, between all staff members, and between all
participants.
vii. Excellence: By fostering good governance, higher level of transparency,
trust, and good stewardship
4. Goal: We will provide affordable housing options through our stepped housing program
to those who have become homeless, as well as those at risk for homelessness after aging
out of the foster care system.
a. Outcome Objective: To reduce the homeless population of foster care age-outs in
Mecosta and Osceola counties by 10% (avg. 22 children per year), by January 1,
2025 (Kids Count Data Center, 2015).
b. Process Objective: Working with existing programs within the community such
as the Michigan State Housing Development Authority and Ferris State
University, we will lay the foundations for independence.
5. Goal: We will guide each participant in setting realistic and attainable short term and
long term goals.
a. Outcome Objective: To assess the weaknesses and strengths of each participant
within the first 24 hours of admission, and to encourage and empower each youth
to become independent within 2 years of admission.

b. Process Objective: Utilizing evidence based tools; we will assess the current
goals and encourage participants to set reasonable and attainable short and long
term goals.
6. Goal: We will equip each participant with the resources necessary to reach their goals.
a. Outcome Objective: To obtain at least 60% successful completion of the
program and reach independence within 2 years.
b. Process Objective: By educating each participant in educational and community
resources, programs, and scholarships as well as principals of resume writing, job
searching, budgeting, life, and social skills we will prepare them for independence
by discharge.
7. Goal: Through continued outpatient services we will support and strengthen the
independence gained within the inpatient program.
a. Outcome Objectives: To achieve a rate of at least 80% that will maintain
independence and be discharged from outpatient services within 5 years of
graduation from the inpatient program.
b. Process Objectives: By continuing to provide supportive services, education, and
counseling until the time of discharge, we will enhance each participants chance
of success.

9. Job Descriptions
a. Board Positions
i. Chief Executive Officer (CEO): As a nonprofit organization, the CEO is
the head of the company and reports directly to the office of the governor.
1. Job Responsibilities:
a. Lead, guide, direct, and evaluate the work of the secretary
and treasurer in accurate and timely execution of their job
duties.
b. Solicit advice from the board of directors as appropriate.
c. Formulate and implement the strategic plan that guides the
direction of the organization.
d. Oversee the complete operation of the organization in
accordance with the strategic plan.
e. Evaluate the success of the organization as well as maintain
awareness and interventions for the organizations
weaknesses.
f. Demonstrate the leadership necessary to reach the goals set
forth in the companys mission.
2. Position Qualifications:
a. Must have a minimum of a Master of Science in Nursing
Degree and be licensed to practice within the state of
practice. A Masters in Business Administration would also
be appropriate but is not required.
ii. Board Secretary: The secretary will report to the CEO and will maintain
board meeting minutes and distribute updates and policy changes to staff
as appropriate.
1. Job Description:
a. Attend all board meetings and take accurate minutes
reflecting the organizations progress, changes, and goals.
b. Have an up to date copy of the bylaws available at every
meeting and assist in ensuring the bylaws are followed.
c. Keep record of company employees and board members.
d. Manage the schedule of the CEO and answer telephone
calls related to the organization during designated regular
business hours.
e. Maintain files of past and current participants by making
following up calls at the designated intervals and updating
necessary information to maintain contact.
2. Position Qualifications:
a. High school graduation or equivalent plus 5 years
secretarial experience is required to gain this position.
iii. Treasurer: The treasurer is in charge of all the financial risks, activities
and responsibilities of the entire company. The duties of the treasurer
include:

a. Analyzing the company's financial strengths and


weaknesses.
b. Working on plans of improvement, financial planning and
managing the company's cash flow.
c. Keeping accurate record of accounts of monies for all
income as well as expenditures for the sake of bettering the
organization.
d. Compliance with audits as requested by the board of
directors and has the ability to be accountable for spending.
e. Preparation of the treasury report prior to each board
meeting to be made available to the board.
2. Position Qualifications:
a. A minimum of a Bachelors Degree in Accounting or
Finance and 3 to 5 years public accounting experience.
CPA is preferred as well.
b. Employee Positions
i. Program Coordinator: The Program Coordinator is responsible for
overseeing the entire program.
1. Job Description:
a. Responsible for evaluating policies and procedures in
place.
b. Must take concerns related to policies and procedures to the
board and make recommendations for change if necessary.
c. Will maintain knowledge of a general plan of care for each
resident.
d. Will set short and long term goals for the program and have
resources available to help GLIDE the residents.
e. He/she should have skills that include analyzing,
communicating, and decision-making.
f. Calculation and distribution of weekly/biweekly payroll.
g. Will operate within the assigned budget for the program
while also reporting budget use and projections at each
board meeting.
2. Position Requirements:
a. Must have a minimum of a Bachelor in Science of Nursing
Degree and a License to practice within the appropriate
state.
ii. Secretary: The Secretary is there to assist the residents, staff, visitors, and
callers with any information and greet them with a friendly smile.
1. Job Description:
a. Answer the telephone during regular business hours and
transfer calls to the proper persons when appropriate.
b. Will keep a schedule of all meetings, events, and details of
the program.
c. Will be responsible for keeping office inventory and
making needed orders.

d. Will stay within the allotted budget when ordering supplies


and report monthly spending to the program coordinator.
2. Position Qualifications:
a. High school graduation or equivalent is required and 3 to 5
years secretarial experience is preferred but not required to
gain this position.
iii. Case Manager: The case managers will work one on one with each
resident to ensure successful completion of the program.
1. Job Description:
a. Manage intake, create care plans, assess goals and barriers,
assess readiness to move from dependent to independent
care, and ability to discharge to outpatient or discharge
from the program.
b. The case manager along with the program coordinator will
assess the need to expel a resident from the program for
misconduct that would warrant termination of services.
c. The case manager may also, along with the program
coordinator, assess if a residents needs could be met with
additional time on a more dependent unit.
2. Position Qualifications:
a. Must have a minimum of a Bachelor in Science of Nursing
Degree and a License to practice within the appropriate
state.
iv. Resident Care Staff:
1. Dependent Care Staff: The dependent care staff will support the
residents in following the rules and regulations of the organization
and its mission.
a. Job Description:
i. Assist the residents in meeting their individual goals
and follow the care plan designed by the case
managers.
ii. Report misconduct accurately and in a timely
fashion to case managers and staff appropriate to
the residents care team.
iii. Supervision of the safe and responsible use of the
kitchen.
1. May answer questions relevant to their
scope of practice and refer to the dietician as
appropriate.
b. Position Qualifications:
i. Minimum of a high school diploma or equivalent is
required. Certified nurse aid training is preferred.
2. Independent Care Staff: The independent care staff will also
support the residents in following the rules and regulations of the
organization and its mission.
a. Job Description:

i. Continue in supporting the residents in meeting the


goals of the care plan.
ii. Hold residents to a higher standard of responsibility
and self-sufficiency as they move toward
independence.
iii. Be supportive and assertive in overseeing that the
rules of conduct are adhered to appropriately.
iv. Report misconduct accurately and in a timely
fashion to case managers and staff appropriate to
the residents care team.
b. Position Qualifications:
i. Minimum of a high school diploma or equivalent is
required. Certified nurse aid training is preferred.
v. Dietary:
1. Registered Dietician: The registered dietician will be a supportive
staff member in assisting the residents to GLIDE into
independence.
a. Job Description:
i. Provide education regarding proper dietary habits
and making a food budget.
ii. Provide healthy cooking classes and printed recipes
and/or cookbooks as appropriate for beginner and
intermediate levels.
iii. Be a reliable resource for assisting residents in
living a healthy lifestyle.
b. Position Qualifications:
i. Must have a completed an accredited Didactic
Program in Dietetics with a minimum of a Bachelor
degree and a license to practice within the
appropriate state.
vi. Housekeeping: The housekeeping staff will be a supportive staff member
in assisting our residents to GLIDE into independence.
1. Job Description:
a. Clean and maintain any community, office, and corporation
spaces.
b. Oversee the resident's cleaning and ability to maintain their
own living quarters.
2. Position Qualifications:
a. Minimum of a high school diploma or equivalent is
required.
vii. Maintenance: The maintenance staff reports directly to Information
Technology and will perform in a manner that is safe for all of our
affiliates.
1. Job Description:
a. Maintenance of the grounds.

b. Consult with appropriate outside agencies as necessary to


ensure the safe operation of the program.
c. Maintains records of scheduled maintenance procedures
and operates within the budget allotted for the maintenance
department.
d. Will document and report all departmental spending to the
IT personnel each month.
e. Providing routine and extensive preventative maintenance
and repair procedures within the organization buildings.
f. Teach and assist the residents in learning tasks appropriate
to renter responsibilities. These tasks include:
i. Changing a light bulb and smoke detector batteries.
ii. Mounting curtains, blinds, and pictures.
iii. Plunging and unclogging drains and toilets.
iv. Learning appropriate care and usage of appliances.
2. Position Qualifications:
a. Minimum of a high school diploma or equivalent and at
least one year of responsible experience in building and
mechanical equipment maintenance and repair; or an
acceptable equivalent combination of education and
experience.
viii. Information Technician (IT): The IT reports directly to the board and is
in charge of the company's website and maintenance department.
1. Job Description:
a. Supervision of the maintenance department.
b. Ensuring safe maintenance practices.
c. Creation and maintenance of an accurate, appealing and in
inviting webpage.
d. Ensure the page is easy to maneuver through and easy to
find.
e. Establish and maintain the overall health, functionality,
safety, and confidentiality of all networks and computers
within the organization.
f. Operate within the established budget for the IT
department.
g. Maintain accurate records of all maintenance jobs and
spending.
h. Prepare documents and written financial reports for the
department to report to the board at each board meeting.
2. Position Qualifications:
a. Must have a minimum of a Bachelor Degree in Computer
Science with A+, and Computer Networking Service (CNS)
certifications.
