Presenting Problem
Craig self-referred as a result of depressive feelings. He feels constantly overwhelmed
with life and reports trouble sleeping and concentrating. Craig says he often questions if he has
made the right choices in life and whether he is following the right path.
There are many stressors that Craig feels could be contributing to these depressed
feelings. One current stressor that Craig discussed is his intense training within the military.
Craig is currently active duty in the Navy and undergoing intense training in an attempt to enter
Special Forces. He mentions that he often does not know when or if he will be going to another
school. His training has been put on hold numerous times due to government shutdown and
budget cuts. He reported that training it is physically taxing. He expressed that he faces a lot of
stress from his work and training in the military. Along with this, Craig also reported that he has
not had the opportunity to take leave and visit his friends and family back in Michigan in the past
seven months. He shared that he feels very alone and that he believes he may be losing contact
with his support system back home. Craig reported that he has a serious girlfriend whom he has
not seen in about four months due to his rigorous training schedule and he is concerned that their
relationship may be deteriorating.
Family of Origin History
Craig reported that his mother was in and out of his life while growing up. When Craig
was three years of age, his grandmother was granted custody and began raising him herself.
Craig reported that while growing up he had a positive relationship with his grandmother;
however, when Craig was fifteen years of age his grandmother sent him to Michigan to live with
his uncle while she worked through a divorce.

Craig reported that his mother was twenty-two years of age at his birth. He denied
knowledge of his fathers age due to the minimal contact within their relationship. Craigs
parents never married.
Craig reportedly has two maternal half-sisters, whom he has minimal contact with. He
reportedly believes Devan* to be around fifteen years of age and Sahara* to be around ten years
of age; however, he is unsure about exact ages. Craig reported that none of his mothers
pregnancies were planned. He also states that his mother has never been married, and therefore,
was not married to any of the childrens fathers. Craig is not sure of the education levels of his
sisters; however, he would guess that their fathers have kept them on track and in school.
Craigs mother is currently forty-six years of age, as of 2014. He reported that his father
may be approximately forty-nine years of age. He doubts that his father attended college;
however, he believes he graduated from high school. Craigs mother dropped out of high school.
Craig reported that his home atmosphere was volatile when living with his mother as a
young toddler; however, he remembers a comfortable, happy, and stable environment when
living with his grandmother. Upon moving to Michigan, his home life was good in the beginning,
but eventually deteriorated when his uncles child was born and Craig became somewhat of a
built-in babysitter as he explained it. He also shared that there was many arguments between
his uncle and the girlfriend, causing a lot of tension within the household. Craig became quite
uncomfortable with this living situation and moved into the spare bedroom at his girlfriends
house, thus living with his girlfriend and her parents his final year of high school until he left for
the military two years later.
Craig reported that his grandmother always made a big celebration of holidays. She
enjoyed cooking meals and having her family together to celebrate. Upon moving in with his
uncle, the celebrations were less immense.
Craig reported that he remembers facing some physical abuse and neglect while living
with his mother as a young toddler. He stated that his mother was often busy with boyfriends and

did not pay much attention to him. He also shared that some of the boyfriends became abusive
with him when he would cry for attention from his mother. Craig reported that although it was
hard to be away from his mother, he was grateful to feel the security of his grandmother.
Developmental History
Craig was born at approximately seven pounds to a full term induced vaginal pregnancy
with no significant issues at birth. He was bottle fed with no know feeding issues present.
Craigs grandmother was concerned that he was facing some developmental delays due to his
mothers lack of nurturing care. Craig was reportedly a very quiet child and did not start talking
regularly until around age three. He was not potty trained until around three years old. Craig
began walking before one year of age.
Health History
Craig had chicken pox as a child, but denied knowledge of any serious childhood
illnesses. He reported surgery on a broken wrist as a child, and later an appendectomy during his
adolescence. Craig was hospitalized in 2012 due to being sick with pneumonia.
Relationship/Sexual History
Craig denied trouble with friendships as he always maintains friendships rather easily. He
reported it can be hard to stay connected with friends due to his constant moving between
training sites as well as the distance between himself and long-term friends. Craig is a
heterosexual male interested in women. He began dating in a serious manner at age fifteen and
had his first sexual experience at age sixteen while dating his second serious girlfriend.
Currently, Craig has been in a serious relationship with current girlfriend for five years and he
enjoys the relationship very much. Craig reported that he is able to enjoy sexual activities and
does not encounter any problems having orgasms. He also shared that his girlfriend is currently
on a birth control pill and they tested negative for all sexually transmitted diseases since they
have become committed to one another.

Craig is currently in a committed relationship; however, he is not married. He began


dating his current girlfriend at age eighteen and they have been together for five years. Craigs
girlfriend was seventeen when they began dating and is currently twenty-three. The couple
currently does not have any children. The nature of the relationship has been good; however,
Craig is expressing some concern with the nature of the relationship lately as he has been very
busy with training and has not had as much time to devote to his girlfriend and their relationship.
Craig mentions that his break ups in the few serious relationships before this current one were
simply because he was young and things just did not seem to be working out. Craig reported that
his girlfriend has recently graduated with her masters degree in communications and is currently
searching for a job.
Substance Use/Abuse History
Craig denied struggles with any substance use or other addictions. He has never smoked
and only occasionally drinks. He denied the need for any addictions treatment.
Mental Health History
Craig reportedly faced some depressive moods in the past; however he never received
treatment for his feelings until now. He denied previous medical treatment for mental health
issues.
Military History
Craig is currently enlisted in the United States Navy and has been enlisted for three years.
He denied combat experience. Craig has hopes to become a Special Forces Operator, requiring a
significant amount of additional training. He reported he should complete his training in fall
2014 and may be deploying as early as January 2015.
Financial History
Growing up, Craig states that his grandparents were not incredibly wealthy; however,
they were able to provide everything he needed as well as many extra things that he desired.
Craig was reportedly well cared for and did not want for much while growing up. Currently, he

reported that he is doing rather well financially and is able to pay all his bills and still have
money left for additional desires and activities.
Legal History
Craig reportedly has not faced any legal issues and has never been arrested or placed on
probation.
Immigration Issues
There are no immigration issues with this client. He is a legal United States citizen and
was born and raised in the United States.
Plan of Change
The main goal for Craig in this situation is to improve his mood and reduce his feelings
of depression. Craig would also like to explore his feelings further and work with a counselor to
develop some additional ways to handle other unresolved feelings.
Some additional goals for Craig include: work through his feelings of concern about their
relationship with his girlfriend, and find an extracurricular activity that will allow him to relieve
stress he is facing from his intense training.

Craig will work to improve his mood and decrease his feelings of depression.
A. Craig will continue to see a counselor.

Craig will see a counselor once a week for one hour at a time.

Craig will continue this counseling for at least 12 more weeks, and
then undergo an assessment to see if further sessions are needed.
