Bio-Psycho-Social Assessment
Latrice Hart (pseudonym) is a 24 year old single African American female who is currently
living with her sister and sisters girlfriend. She just became recently employed at a group home
as a Direct Care worker for developmentally disabled women. Also, she is currently enrolled at a
community college. She is a mother of one child (male, 6 months old). She doesnt get along
with the sisters girlfriend. They always have little disagreements due to the girlfriends smart
remarks that she makes. Latrice feels that she is being forced to get along with her due to the fact
that she is her sisters girlfriend and she doesnt want any tension between her sister and herself.
She would like to move out into her own place. Her sons father is vaguely in his life because of
his current lifestyle. Ms. Hart comes to the social worker because she feels like she is going to
have a huge argument with the girlfriend and it is stressing her out.
Family of Origin History:
Her mother was 23 years old and her father was 22 years old at the time Ms. Hart was born. The
couple werent married at the time of birth. The pregnancy was accidental. The current age of
mother is 48 years old and father is 47 years old. The client has 4 siblings (Trevor, 28, Patricia,
22, Taylor, 20 and Pier, 15). All the siblings are single except for Trevor and Taylor, both are in a
committed relationship. The parents separated due to discrepancies that couldnt be solved
between them both.
The atmosphere of the household was hostile. Mother was angry at the client for telling the
grandmother what happened in the home. Mother always told client, what happens in my house,
stays in my house, it is no one else business in what is going on in here! Client received
spankings as a punishment when she told her grandmother about the things happening in home.
Holidays in home were very fun, filled with lots of love, laughter, and food. The concept of death
was explained that the deceased person is being called home to the Lord. Client and siblings (at a
young age), would ask if they could go see the Lord with the deceased person and family told
them no.
The educational level of the mother is she graduated from high school and attended college but
didnt graduate. The father didnt complete high school. The first sibling graduated high school
and now attends college. The second sibling graduated high school but didnt attend college. The
third sibling graduated high school and attends college. The last sibling is a high school
sophomore.
There was no physical or sexual abuse in the home. Client specified that the father denied being
her parent for a long time.
Developmental History:

The length of the clients birth was the full 9 months. She weighed 7lbs 5oz. The client was a
healthy baby at birth. She was delivered vaginally. She was breast fed at first, then was switched
to the bottle. No feeding issues. She was weaned off the bottle at the age of 1. She began walking
at the age of 1. She began speaking at 7 months old. She was potty trained between the ages of 2
and 3. No development delay except for potty training.
Health History:
Ms. Hart was healthy as a child. Only issues she had was asthma and eczema. As an adult, no
major health issues. She had a C-section for her last pregnancy. She had 4 miscarriages prior to
her last pregnancy. Started her menstrual cycle at the age of 9. No known allergies to any
medications but is allergic to latex products. She had two sexually transmitted diseases
(trichomoniasis and chlamydia).
Relationship/Sexual History:
Ms. Hart have 2 close childhood friends. Right now, she was 3 close friendships. The duration of
the first friendship is 12 years. The duration of the second friendship is 11 years. She began
dating at the age of 16. She is heterosexual. Her first sexual experience was at the age of 16. She
has had the ability to enjoy sex. She has 3 current sexual relationships. She stated that she was on
birth control until she could not afford the prescription fee due to no job. She is currently waiting
on the pharmacy to transfer her prescription to a closer pharmacy to her. She uses protection. She
had contracted 2 sexually transmitted diseases (see health history) and was treated for both.
Family of Creation History:
Ms. Hart was in four committed relationships. She did not marry any of the people in her past
relationships. The age of her partners at the beginning of the committed relationships were 16,
18, 19, and 20. Out of her last relationship, she have given birth to her son Armani and he is 6
months old. The level of satisfaction in the first relationship was very satisfying and for the
others, they were mildly satisfying. There were four breakups. The reason for the first break up
was the mother hated the partner for unknown reasons. The reason for the second break up was
the immaturity of the partner. The reason for the third breakup was the personality of the partner
was terrible. The reason for the last break up, both partner and her werent on the same page. The
educational level of the first partner is a graduate of high school and was studying carpentry for a
trade. For the second partner, he graduated high school and became a Certified Nursing assistant.
For the third partner, he graduated high school and currently attends college. For the last partner,
he graduated high school and attended college but didnt complete it.
She had five pregnancies. Out of her pregnancies, she had four miscarriages and one full term
pregnancy. She havent had any abortions. No planned pregnancies. There was no physical abuse
in the relationship. There was only verbal abuse in the last relationship but it was very minimal.
She have a great relationship with her child. There is no relationship with none of her partners,
except for the last partner. They both co-parent.
Substance Use/Abuse:

Ms. Hart smoked marijuana. She began smoking at the age of 17 while visiting a cousin in New
York. February 10th, 2014 was the last time she smoked. There is no addiction to the substance.
There is no addiction to cigarettes, eating, nor gambling.

Financial History:
Her family finances while growing up was steady but it was enough to maintain a household.
Currently, she is almost stable financially. She just began working on 2/5/14.
