Kindly please note these documents contained here in the envelope, these are the

corrections and the additional information

1. I have with me the unofficial receipt of our rental payment, but I have attached wherein
the certification of our monthly rent from our lessor.
2. There are no attached water bill charges, for we dont yet piped water inside our house.
We are relying on poso negro or a manual water pump which is found in our apartment.
3. I have no permanent residence in Manila, so I didnt attach here the vicinity map form.
But I have expressed my desire of renting a room within the 200 to 300 meter radius from
your University. My main mode of transportation will be walking.
4. We have tricycle but it is not fully paid. My father is currently paying for it quarterly so
we didnt really consider it a family vehicle because it is used only for his work.
5. I have an attached official receipt of our monthly electric bills from December 2014 to
February 2015.
6. I have with me a Certificate of Indigency exempting my parents from paying their taxes
to the government. Their combined earnings are below the minimum requirement set by
the BIR for them to pay it.
7. The name indicated in our electric bill is the last person who rented this apartment. The
energy provider here in Masbate, the MASELCO (Mastbate Electric Cooperative) didnt
change the name since then, but these electric bills are ours, we personally payed for it.
