1- icon in submit button is not suitable and the icon not represented as a subm

2- "Medicine Name" text font is style not same with other because it using bold
style and it not consistent with other text.
3- There are has large gap between "Doctors Name" text field and "Medicine Mane"
select box and this will lead to wasted the space.
4- the text field on "Your name" is large form other such as text field "Doctors
Name".
5-The star(**) after "Date" text and star(**) before "Use MM/DD/YYYY" is mistak
e because it sometimes difficult to identify.
No. 1(icon submit button)
- use the submit button that suitable to represent it and no icon on submit butt
on also suitable.
No. 2(bold text font)
- the text font must same with other to make user less confuse about system.
No. 3(Gap space)
- the gap must maintain with other field to save the size page such as "Doctors
Name" text field and "Medicine Name" Select Box.
No.4(Large Text field)
- the text field must maintain with each other text field to make design look in
teresting such "Your Name" text field and "Date" text field.
No.5(star [**])
- the condition field and information about condition should closer to each othe
r because it will become easy to user refer such as star(**) before "Use MM/DD/Y
YYY" text located below star(**) after "Date" text.
