Purpose: The purpose of this assignment is for you to begin to address the graduation
requirements for your major and to address the entrance requirements for your chosen four-year
institution. Please address the questions below. Please type your responses.
1. What courses are you currently taking this semester?
I am currently taking COMM 101 C19H, EDUA 240 CO1W, EDUA 245 CO2W, EDUC 290
CO5W, and MATH 257 CO4W.
2. What courses do you need to graduate with your AA degree? (Please see Lena Paxton to get a copy

I need to take a GEN. ED. Physical Science, MUSI 233 Music Methods/Elem Teacher,
PHYE 155 Health & Wellness, , EDUA 248, and EDUA 231.

I have not applied for CSI graduation since I still have the Spring 2015 and Fall 2015
semesters to complete.
4. Have you completed CSIs Computer Literacy Assessment? (http://computerliteracy.csi.edu/)
I met the Computer Literacy Assessment because I completed EDUC 215 in the Fall 2014
semester.
5. What are the entrance requirements for your chosen four-year institution? What are the
application deadlines? If you plan to apply for scholarships, what are the scholarship
application deadlines? Have you applied for financial aid?
The entrance requirements for Idaho State University are as follows: I must have a
cumulative grade point average of 2.75, complete or have in progress EDUC 202,204,205,
and 215 or equivalent, ENG 101 or ENG 102, COMM 101, MATH 157 or 257 of which only
one course can have a C, have an FBI Background Check completed or in progress, Praxis
1 scores must be a 165 in Reading, 174 in Writing, a 164 in Math and completed before
October 1st for Spring Admission, Admittance to the university with a $50 fee, complete the
application to the Teacher Education Program, a copy of my unofficial transcript, a copy of
my current class schedule, an EDUC 202 form, and an interview. The application deadlines
are March 1st or June 1st for the Fall Admission or October 1st for the Spring Admission. The
scholarship application deadlines are February 15, 2015 for the Fall semester and November

1, 2015 for the Spring semester. I will be applying for financial aid in February 2015 once I
get my taxes done.
6. Does your chosen four-year institution require Praxis 1 (or other tests)? What scores are
needed? What are the dates for Praxis 1 (or other tests)? Do these tests have to be completed
before the application deadline? If so, how long does it take to process test scores?

Idaho State University does require the Praxis 1 test for Elementary Education. I must
have the minimum of 165 in the Reading and Language Arts Subtest (5032), 164 in the
Mathematics Subtest (5033), 155 in the Social Studies Subtest (5034), and 159 in the
Science Subtest (5035). The dates for Praxis 1 tests are 1-12-2015 to 1-24-2015, 2-92015 to 2-21-2015, 3-9-2015 to 3-21-2015, 4-6-2015 to 4-18-2015, 5-11-2015 to 5-232015, 6-15-2015 to 6-27-2015, 7-13-2015 to 7-25-2015, and 8-10-2015 to 8-22-2015.
The Praxis 1 test must be completed by August 31, 2015. The test results are immediate
once you complete the test.
