Checklist
Read this entire packet, including the supporting materials.
Confirm that you will enter the program by returning the Statement of Intent form or email to the Admissions and Records office if you have not yet done so. Failure to do so will
affect your ability to register.
Complete the University of Hawaii Medical Information Form
(http://www.hawaii.edu/myuh/healthform) and fax to (808) 956-3583. The deadline is
December 2.
Make arrangements to attend ETEC 501 (orientation) on Friday, January 9, 2015.
Check your UH username status by visiting:

Email the Field Placement Coordinator, Dawn Paresa (deparesa@hawaii.edu) regarding
fingerprinting and the field component of your program.
Email the Special Education academic advisor, Rumi Heine (rumi@hawaii.edu) about your
course schedule.
Register for each course per instructions once Fall CRN number(s) are available. CRN
number(s) will be e-mailed to you at a later date. **Note: Your advisor will work with you
on outlining your semester and will assign set courses to you.
Pay for courses.
Apply for Financial Aid and Student Loans (see information enclosed) if needed.
Read through and complete all statewide Tasks to Complete.
