Actual Implementation

First Visit
June 26, 2014
In our first visit on Managok we arent able to conduct assessment and interview due to
limited time, because we were too busy talking to the purok and zone leader, finding a perfect
spot for the upcoming macro teaching and also visiting the areas that was on the survey.
In this day, we focused on preliminary dissemination of information regarding the macro
teaching. On the next day we gathered all the datas that was collected during the 2nd sem of
2013 and 2014, analyzing and interpreting the data collected and completing the program and
topics that will be discuss on this coming Friday July 4, 2014.

Second Visit
July 3, 2014
On the second visit we disseminate the information about the macro teaching and
distribution of the flyers in each houses. We also assess and survey the family that we choose.
We choose that family because they were under the required qualities for the care study.
Since second day was the first meeting with the family, we focused more on establishing
trust and rapport with the family members. We introduced ourselves and explained the purpose
of our visit and succeeding visits. A simple physical assessment was also done and we were
able to impart health teachings to them. On the next day we conduct macro teaching in
Langasihan Basketball Court about family planning.
Third Visit
July 11, 2014
During third visit, we continued assessing the family and identified health threats and
provided them with additional health teaching. We also took pictures of the area and the family
members. We were able to collect data needed stated in the assessment sheet. We also
observed the environment and identify specific applicable problems in their area.
