Dear Fellow Employee,

According to the policies and procedures set forth in the

Pennsylvania Code Book, Chapter 3290 Family Child
Day Care Homes all employees must obtain a child
background clearance, criminal history check, FBI
clearances, and physical along with a tuberculosis
screening. Attached, I have included all forms, on each
form you are provided instructions on how to complete
each form. All paperwork must be completed by
September 15
th
2014. You will not be able to return to
work after this date until all paperwork is returned by
the state and physical with tuberculosis results are
submitted. If you have any trouble during the process
