ACKNOWLEDGEMENT

Primary Health Care stands for taking good care of ones family. It is an essential health care, based on
practical, scientifically sound and socially accepted methods and technology, made universally accessible
to individuals and families in community. Is an approach to health beyond the traditional health care
system that focuses on health equity-producing social policy. Primary health care states the health of
others and their problems and people who are in needs. On this case, we are blessed that we
successfully accomplished our case study. First of all, We would like to thank our Almighty Father who is
always there for us. He, who sent forth his holy spirit to guide us, protects us in our all ways. The one
who give us strength to pursue our tasks. Without him, we could no longer finish our case study. By his
helping hand, we accomplished our studies by his guidance. Second to thank is our beloved parents who
are there to support us for our studies and also in our financial matters. The one who give us more
strength and encouragement to fulfill our daily tasks. The third one to thank is our Clinical Instructor
Mrs. Emily C. Albacete RN, MN. Because she is always there to help us in our case study to make it
presentable. She is approachable when we are in problems. The time that we are having a community
service in Anakan, Gingoog City she guides us always and never let us stand alone by ourselves. She who
is willing to help when we are in difficulty in problems. We are very thankful and blessed that she
became our Clinic Instructor. Weve learned a lot of her. Lastly to thank is our co-members because we
help each other and united as one to be able to accomplish these kind of tasks. We encountered lots of
difficulties but despite of it, we achieve the trials
