On Thursday 26 June 2014 at 10:00 hours, the forklift I was reversing

side swiped the left side of the uggy parked eside !arpentry shelter
in the area of "aydown I# $earing the sound and yell for attention
fro% a !o& worker of %ine, I reali'ed that I side swiped a uggy# The
forklift was not !arrying any %aterials so I thought I did not need any
spotter#
On Thursday 26 June 2014 appro(i%ately at 10:00 the uggy that I
parked in area laydown I east side !arpentry shelter was side swiped
y 16&ton )orklift $yster when it was reversing# *onse+uently, it
!aused a little da%age at the left side of the uggy& the seat handle
and %inor s!rat!h in parti!ular# I was inside the shelter talking with
the !arpentry tea% when suddenly I heard a sound# $owever I was
not aware if the forklift side swiped the parked uggy# ,o one got
