Therapeutic communicate with patient

Perform complete mental status assessment
Summary of activities:

Helps patients with breakfast and lunch.
Assessing the patients skin. Observe that Some patients scratch the skin when anxious.
Helping with other staff to set up the dining hall before and after feeding. Medication sheet
evaluation. Obtain of Vital Signs. Complete physical assessment, and focused mental status
assessment.

Experiences
My good morning went very well, everybody answered. Every patient at Meadow Brook was
quite happy this morning.
My patient is breakfast person. She was happy to see the breakfast .but ate 60 % of breakfast..
She ate almost 50% of each meal, which is above average for her. Her mental and physical
assessment was difficult to obtain, because she dont know English, and dont respond very well.
She is under weight but very energetic and wants to move from one room to another. I had good
feeling to see that her husband visit her daily. Overall, It was good clinical day.

Gained Critical thinking and congenital skill when dealing with depressed and anxious patient.
Exceptional understanding in various ways how the Nursing theories has relevance for clinical
practice. Gained the knowledge that how therapeutic communication really does affects on
patients behavior and thinking.
