Greetings! May I have the honor to apply for promotion from midwife II to midwife III in Wao
Rural Health Unit, Wao District Hospital, Wao, Lanao del Sur.

Ive been in the service as Midwife II in RHU Wao for 36 years of which I believe I am qualified
as Midwife III, being the next in rank employee in line with this position which I am applying for.

My educational qualification and attainment are as follow. I am a graduate of midwifery course
at Iloilo Doctors Hospital Iloilo City March 1976 and passed the Midwifery Board Exam. May
1977 grade of 75%. Serving the RHU Wao, as Midwife II from April 1978 up to present with very
satisfactory performance.

If given a chance to be promoted as Midwife III, I will be of service to the people of Wao
neighboring and barangays by competently providing DOH service in RHU Wao not only
nationally but globally.
