PETITION FOR GRANT OF LETTERS OF AFTER TWENT-ONE YEARS

PURSUANT TO 20 Pa. C.S. 3152

The Petition of (JOANNA MURPHY), (GRANDDAUGHTER to Estate), respectfully

states that:
1. Petitioner is the (Granddaughter) of (George Cook), who died intestate on
(12/06/81) a resident of Philadelphia, Pennsylvania.
2. At the time of his death, the decedent owned property in Philadelphia,
Pennsylvania.
3. Petitioner is unaware, after reasonable research, of the existence of a Will
written by the decedent.
4. Pursuant to Section 20, Probate, Estates and Fiduciaries Code, 20 Pa. C.S.A.
Sections 101 et. Seq. petitioner is an intestate heir.
5. Petitioner is aware of other intestate heirs, who have been notified of the filing
of this Petition as indicated in the Certification of Service with the notice
letter, attached hereto as Exhibit “A”.

WHEREFORE, petitioner respectfully requests this Honorable Court to

Order and Decree that and estate may be open after 21 years for the purpose of
administering the estate.

DECREE

AND NOW, this 17th day of November, 2009, upon consideration of the
annexed petition, it is hereby ORDERED and DECREED that the Registered of Wills
is authorized to grant letters of administration upon the Estate of (GEORGE COOK),
Deceased, to person entitled thereto.
