Meeting

Overview that we listed off in the beginning of meeting:
SA discuss today
JK in planning phase
JR discuss today, King has plan, but needs updated diagnosis and
documentation.
DC Raketty does not have parent/teacher info.
KV discuss today, agreed on plan
KR discuss today, needed to tweek some accommodations
OJ discuss today
VL done
CW discuss today
CB discuss today
MH Gravelle requested for staff feedback
RG Gravelle added and discuss today. Gravelle has plan, need to sign 504

Notes
SA: Started SIT evaluation for depression. Recent sleep study suggests a
circadian disturbance. Doctor says to please allow a later start time. Raketty
has referral, notice of action, and consent signed. Raketty will work on plan.
She already has a late start she starts 3
rd
period. Last semester she only
passed sociology. Raketty would like to do extra time for tests and homework
in terms of accommodations.
JR: Is being referred to sped, but King has worked on a 504 plan in the mean
time. Her wording is worded very nicely, we passed around for signatures
can be moved off.
KV: Looking at 504 for depression and migraines. Taylor met with both
student and mom and found out that Kira is not doing well. Team agrees that
she needs a 504. Accommodations: 1) wants the opportunity to receive
instruction, when available and when absent during instruction time, 2)
ability to make up work with no penalty due to illness, 3) frequent
encouragement and reassurance of positive academic achievements, and 4)
refrain from asking her to tutor or assist other students as this creates undue
pressure. Sending determination around for signatures. Sending the plan
around for signatures I believe this can be moved off the list, if you agree
with actions.
KR: King added being able to test in a separate location to his 504.
Determination and plan is being sent around for signatures. I believe he can
be moved off the list as well, if you agree.
OJ: Kleckley hasnt heard from Olivia and voice message has been left for
mother. She has sent for staff feedback and we will hopefully be able to
evaluate at our next meeting on the 17
th
.
CW: Taylor knows he is struggling academically. Taylor will read emails and
find out what is going on.
CB: Has had 4 or 5 concussions, which are really affecting her. Doctor says
she should have her PEs waived and have a 504 plan. She has difficulty
concentrating and with her memory. Right now she is sitting in SIT, but
Raketty will contact parents, have discussion, and get consent signed. Team
agrees that Cheyana should have a 504 plan.
RG: Gravelle met with parent. Plan includes extra time for homework and in
class work, providing make up work when absent, and extended time on
tests. Plan was sent around for signatures. Can be moved off the list.
