1. Describe the organizations mission, goals, and programs/services.
The Lincoln County Health Department provides quality health services to promote a healthy
community. Our departments mission is to promote healthy lifestyles through prevention,
preparedness, and education. We provide health services for children and adults including
communicable disease investigations and HIV/AIDS testing. The General Clinic offers
immunizations and pregnancy tests; counseling and education regarding family planning and safe
birth control methods including administration of birth control methods; prenatal care including
medical services for Medicaid and/or indigent pregnant women; WIC which provides nutritional
education and food supplements to low income, pregnant, breast feeding and post-partum women,
infant and preschool children identified as nutritionally at risk. We provide educational programs
to schools and organizations as well as assist in the Lincoln County Health Assessment every four
years. Since 1997, we have received a grant from Smart Start, a North Carolina award winning
early childhood initiative that is working to ensure that all children have the experiences they need
to thrive. Since 2004, we have received grants from the Susan G. Komen foundation in the fight
against breast cancer in Lincoln County. We are currently collaborating with surrounding county
health departments in a $2 million grant, funded by the Centers for Disease Control and
Prevention, entitled North Carolina Community Transformation. This project is working to make
healthy foods and physical activity more available in communities, as well as working to help
combat the rate of chronic disease in adults. Our department was selected to serve as the Healthy
Food Environment Lead and in just under a year, six new farmers markets have been opened.













2. 2. Provide a description of the project including key strategies, timeline for project.
The goal of the Learn and Serve program will be to lower sexual risk behaviors in Lincoln County
adolescents. Learn and Serve is structured from an evidence-based program that has shown a
median relative decrease of 39% in the rate of pregnancy. In order to promote enthusiasm and
engagement in activities there will be t-shirts distributed to each girl who has successfully
completed the program. As an incentive to join the program, each participant will be entered into a
drawing for a mini iPad. The Health Educator of Lincoln County Health Department will first
attend a 3-day training session in Washington, D.C., Advocates for Youth, in order to prepare for
the teaching portion of Learn and Serve. Upon return, an appealing, promotional brochure will be
mailed to community leaders, and a flyer will be posted at the high school for students to review
and sign up for the program. A power point will be presented before the school faculty and
community leaders to promote the Learn and Serve program. The Learn and Serve program will
be offered to thirty West Lincoln High School 9
th
grade girls as an after- school program. The
Learn and Serve program will meet from August 18, 2014 through December 18, 2014 and will
include two hour class sessions twice per week. The thirty 9
th
graders will be split into two groups,
thus fifteen girls will meet on Monday/Wednesday and fifteen girls on Tuesday/Thursday. These
sessions will include open discussions of chapters in the book, Life Planning Education: A Youth
Development Program. The curriculum includes role-playing, games, art projects, problem-
solving scenarios, brainstorming, videos, guest speakers, and group discussion. Four female
community leaders will be asked to be the guest speakers. The students will gain knowledge about
themselves, sexuality, prevention of pregnancy and sexually transmitted disease, employment
preparation and related topics. They will practice skills of goal-setting, decision making,
negotiating, communicating, and managing stress. On the first Saturday of each month the students
will be involved in a community service program of their choosing for a total of four
opportunities. The community service component will allow for non-parental mentors and an
opportunity to interact with different age groups within the community. Learn and Serve will be
coordinated by the Health Educator of the Lincoln County Health Department. Evaluation of
Learn and Serve will include pre/post knowledge surveys as well as attitude surveys.






3. Describe the program/need that the program will address.

Lincoln County, situated in North Carolinas Piedmont region, is rich with early landmarks and
historic sites which are a constant reminder of the pioneers that came to this area in search of
new opportunity and rich farming. Despite this rich American history, children of this rural
community face impoverished conditions. Of the 79,000 citizens, a third of the children in
Lincoln County under the age of 18 live in households with incomes below the federal poverty
threshold (Census, 2011). The poverty rate for Lincoln County in 2011 was 33.6 percent. This is
more than double the national rate of 15.9% in 2011(Census, 2011). The median household
income is $42,500 compared to the state at $46,500, and the U.S. at $53,000(Census, 2012). The
unemployment rate in Lincoln County currently stands at 8.6 percent.

Poverty is both a cause and a consequence of teen pregnancy and childbearing. North Carolina
was ranked 19 out of 51 (50 states + the District of Columbia) in 2011 teen births rates among
females aged 15-19 (with 1 representing the highest rate and 51 representing the lowest rate). Of
all births to women aged 20 and under, 87% were outside of marriage (hhs.gov). In 2011, a total
of 329,797 babies were born in the U.S. to women aged 1519 years, therefore resulting in a live
birth rate of 31.3 per 1,000 women within this age group (cdc.gov). This is a record low for U.S.
teens in this age group. North Carolina, in 2011, had a birth rate in of 34.9, higher than the
national rate (hhs.gov), and Lincoln County had a teen pregnancy rate of 43.7 (appcnc.org). The
Presidents Teen Pregnancy Prevention Initiative, first funded in 2010, is a very likely cause of
the record low U.S. birth rate. With this grant Lincoln County Health Department will be able to
use an evidence-based youth development behavioral intervention that emphasizes social,
emotional, and cognitive competence training combined with community service in efforts to
reduce the pregnancy rate in Lincoln County.





















4. List the overall goal (s) of the program.

5. List the objectives. These should be measurable.

1. By August 1, 2014, at least 75% of the faculty and community leaders will have been
involved in a power point presentation of the Learn and Serve program.
2. By December 15, 2014, at least 75% of the participants will be able to cite three ways to
prevent pregnancy.
3. By December 15, 2014, at least 75% of the participants will have been involved in four
community service opportunities.
4. By January 1, 2015 a committee will have been established to extend Learn and Serve to
other Lincoln County high schools.
5. By January 1, 2015 evaluation of the Learn and Serve program will be completed.

6. 7. State how, when, and by whom an evaluation will be conducted including any reports
that will be compiled and how the evaluation will be used for program improvements.
Lincoln County Health Department will evaluate the Learn and Serve program by January 1,
2015, to determine whether it met the desired objectives. Evaluations will include:
Number of faculty and community leaders involved in power point presentation.
Number of female participants who were able to recall, from memory, three ways in
which they may prevent teen pregnancy.
Number of participants involved in community service opportunities
Action plan to extend the program to other Lincoln County high schools.
Pre-and post-surveys administered to assess knowledge, beliefs, and attitudes of
students regarding sexual risk behaviors.
In addition to completing the surveys and assessments, the Lincoln County Health Department
will receive a written evaluation by January 3, 2014. The results of these evaluations will be
submitted to the Austin Foundation, the Lincoln County Board of Public Education, West
Lincoln faculty and community leaders. The Learn and Serve program will be publically
highlighted in the school and county newspaper at the beginning of the spring 2014 semester to
demonstrate the effectiveness of the program.

The data collected will be used to illustrate the beneficial outcomes of reducing sexual risk
behaviors after implementation of Learn and Serve. It will be the first of four programs, which
will be expanded to include North and East Lincoln High as well as Lincoln Charter School. The
program will be extended to the other high schools made possible through research and writing
additional grants to the United Way and Carolinas Healthcare System. The Lincoln County
Health Department is willing to provide brochures on teen sexual risk behaviors.
