A. General Data
1. Description of Site
Mid-County TaeKwonDo is located at 1427 S. HWY 69,
Nederland 77627 and the phone number of 409-724-6668. (1)
The business is located in a shopping center with many other
companies. Mid-County TaeKwonDo is a franchise with many
different locations. This location was established in 1984 it
specializes in teaching self defense and developing character in
its participants and is recommended by some of the local
schools for anger management of the children having issues in
the public school systems. This business is funded private and
corporate sponsorships and personal membership fees from
the students and their parents.
2. Description of Target Group
The target group for this site are individuals ranging from the
age of 4 to 16 years of age and includes 70% Caucasian, 15%
Asian, and 15% African American. (2) This target group
consists of 70% female and 30% male. The director stated that
a majority of the target group consists of low-income
individuals from Nederland.
3. Description of Staff Population
The staff of Mid-County TaeKwonDo consists of the owner and
director, five assistant instructors and one office manager. The
staff at this site has an education level of some college. The
responsibilities of the owner and director consists of all
instruction class, clerical duties, attending meeting with the
local public and private schools, setting up tournaments
throughout southeast Texas as well as some held in other
states, and supervising the assistant instructors. The assistant
instructors assist in teaching classes, working the front desk to
sign up new members, make sales and answer questions that
parents would have. The assistants also help to test students,
give awards, teach private lessons, as well as mentor the junior
instructors in how to properly instruct a class. The office
manager is over all front desk operations, accounting and
bookkeeping, inputting clerical data needed for membership
collection data. Making calls to places of business as well as
schools to set up demonstrations and inspirational speaking
seminars. The office manager is also in charge of ordering and
keeping up with inventory.
4. Description of Services Provided
The coordinator of activities is the owner and director of Mid-
County TaeKwonDo Academy who teaches martial arts and
self-defense classes to children 4-16 years of age as well as
adults. Instruction classes are offered Monday thru Saturday
from 4-7 pm, with private lessons offered anytime. Future
plans for this site includes a self-defense clinic open to the
public, a tournament scheduled for March, and rank testing in
April. Daily routine for the director is to open at 9:00am make
phone calls from the prior day, answer emails, attending
meetings with the school districts, instructing class at a local
daycare. He also attends meetings with other area instructors,
and then begins teaching the scheduled classes, ending his day
at 9:00
5. Other Pertinent Data
Water fluoridation for Nederland and Port Arthur is .80 ppm.
The majority of the students bring their own water bottles
from home. There is no drinking fountain available, so many
students either purchase bottled water from the academy or
bring some from home. The children receive no fluoride
benefits from the self-brought water. (3)
B. Information Related to Dental Health
A service regarding dental health is the requirement of the use
of mouth guards during sparring. When the students receive
their mouth guard along with their sparring gear they are
instructed on how to mold the guard to their teeth. They are
also instructed how to care for their guard; when it needs to be
replaced and how to clean it. They are also instructed that the
recommended type of guard would be the use of double mouth
guard. These guards can be purchased at any store but they are
encouraged to be purchased from the academy to ensure that
they are the correct type. There have been no previous dental
programs done at this site. The staff appears to have very little
dental knowledge. The learning level would be unaware. The
academy sells bottled water, monsters, and sports drinks. The
students can also fill their own water bottles in the bathroom
sink. (2)
C. Dental Health Status
1. Dental Caries
a. According to a prevalence study conducted by the Texas
Department of Health Services in compliance with the Center
of Disease Control, 73.3% of Texans have either had previous
caries experience or current decay by the 3
rd
grade. This study
was held in random public elementary schools throughout
Texas during the 2007-2008 school year. (4) Consumption of
sports drinks over a period of time can increase the amount of
time that the teeth are exposed to sugar. With prolonged
exposure you can see an increase of caries.
2. Periodontal Disease
a. According to the Clinical Key website almost all young children
will have gingivitis at some point in their childhood. This
disease prevalence is generally equal between the sexes.
African American has a higher incidence than Caucasian. There
is no genetic predisposition for gingivitis. Lower SES groups
may not have access to dental care. (5)
3. Malocclusion
a. According to the third National Health and Nutrition
Examination Survey incisor malocclusion occurs in the
majority of all ethnic groups with only 35% of the population
having well-aligned incisors. About 20% of the population has
deviations from the ideal occlusion. Incisor irregularity and
both severe Class II and Class III malocclusions are more
prevalent in Mexican-Americans. Over 30% of white youth,
11% Mexican-Americans, and 8% African Americans report
that they are receiving orthodontic treatment for malocclusion.

4. Oral Cancer
a. According to the American Caner Society about 37,000
people will get oral cavity or oropharyngeal cancer. Estimated
than 7,300 will die of these cancers. The average age diagnosed
with these cancers is 62. They can occur in young people but
rare in children. A little more than 25% occur in patients
younger than 55. These cancers are twice as common in men
than in women and equally common in blacks and in whites.
The more common sites where the cancer occurs are the
tongue, tonsils, gums, and floor of the mouth. (7) According to
research smoking use was up to 38.7 % in ages 11-17. (10)
5. Utilization of dental Services
a. Dental care utilization for children ranges from 30 to 74 %
across the United States. In Texas dental care utilization rates
were higher among children with Medicaid than children with
private insurance benefits. (8)
6. Additional Information
a. The Port Arthur Health Department gave out vouchers for
people who could not afford to have dental work done. These
vouchers were discontinued 4 years ago due to a lack of
funding. They gave out 30-50 vouchers every month, to be
used for extractions or any other type of emergency care
needed. (9)
b. Nederland has a water fluoridation level is .8 ppm and Port
Arthur has a level of .8 ppm. (3)
Goal Statement
To develop and implement a program to improve the oral health of the
students at Mid-County TaeKwonDo.
Objective Statement
1. To increase the basic dental knowledge by 50%, measured by a pre
and post-test.
2. Increase the utilization of oral protective equipment to 100%. This
will be measured the site implementing a mandatory use of oral
protective equipment by the students during sparing.
3. To reduce the plaque score by 15% measured by a pre and post
plaque score.
4. To set an age limit on the purchase of sugary drinks measured by
implementation of this policy.
5. To purchase a Save-A-Tooth kit (or equivalent) to have on site in case
of emergencies, to be measured by a follow up call to the Tae Kwon Do
academy.
Rationale for Program
We chose this site because we know the importance of educating the
younger population about oral health and the importance of wearing a mouth
guard for contact sports. We know that if we can reach this target group
early enough to start them on good habits then they will be less likely to have
dental concerns later in life.
Program Design
A. Activities
The program is aimed at educating the students of Mid-County Tae
Kwon Do on basic oral hygiene and the importance of using
protective mouth guards. We will be using the Tae Kwon Dos
constant contact weekly email to invite students and parents to
participate in the oral hygiene program.
Session 1: Each child will take a dental pretest to establish their learning level and
dental health knowledge. With prior parental consent given we will disclose each
child and have them brush in the mirror. We will then modify their brushing to
more effectively remove the plaque. We will discuss plaque, brushing, and caries
process by utilizing three pods that the students will rotate to.
Session 2: We will talk about the importance of using mouth guards while
participating in a contact sport. We will talk about flossing, and gingivitis.
Session 3: We will give each child a posttest to establish if they have had an increase
in dental knowledge. We will also do another plaque score to see if they are using
what we have taught. We will reevaluate the brushing technique and discuss the
importance of fluoride. We will also discuss what sealants are and why they are
important in protecting the teeth from caries.
Session 4: This will be with owner and office staff to discuss the importance of
having a Save-A-Tooth kit (or equivalent) in their emergency kit. We will also
discuss the importance of not having sugary drinks readily available to the students.
We will also discuss the importance of drinking public water versus bottled water.
B. Constraints and Alternative Strategies
1. Parents not giving consent to the use of disclosing
solution. We will have the child participate with all
activities just without the use of disclosing solution.
2. Inability of students to attend at scheduled meeting
time. We will reschedule the meeting time to a later
date.

Evaluation
A. Formative
a. We will conduct a weekly plaque score to evaluate the
effectiveness of our instructions given to the students.
b. Evaluation given weekly by the site director of the
success of the program throughout the process.
B. Summative
a. We will conduct a pre and post-test to evaluate the
learning level of the students.
b. We will evaluate whether or not the site implements the
mandatory use of mouth guards by their students
during sparing.
c. Inspect site to see if they have purchased a Save-A-
Tooth (or equivalent) to help preserve a tooth that is
traumatically exfoliated.
d. Evaluation of the over all success of the program by
having an evaluation done by the site director.

3. CONTENT OUTLINE
a. Plaque: We will show the students a flip chart on the
different stages of plaque. What it looks like and where it
forms on the teeth.

then disclose them. Have them look and see where they are
missing while brushing. Caitlin will demonstrate proper
brushing technique using a puppet and toothbrush. Then she
will pass around the typodont and have them demonstrate
proper technique.

process begins and progresses. We will show them a chart
on how much sugar is in each type of drink available.

a. My name is Ricky Turner, this is Caitlin McFarland and this is
Jennifer Seegers. We are students at Lamar Institute of
Technology in the Dental Hygiene program.
b. Today we are going to be talking about plaque, brushing and
how cavities are formed.
1. As an attention grabber we will show a picture of a
mouth with rampant caries.
c. At this time we are going to hand out a pre test for all of you
to take. This will let us know at what level your dental IQ is
(how much you know about your oral health).
d. After the pre-test we will have the students brush, disclose
and take a plaque score. We will then break up into groups.
Each group will be going through pods with each student
from LIT. Rickys pod will be informative as to what plaque is
and how it forms on the teeth, Jennifers pod will be
informative as to what the caries process and what causes
caries, and Caitlins pod will be demonstrating proper
brushing techniques and instructing as necessary using
models.
1. Group 1 will consist of 4 to 7 year old students

3. Group 3 will consist of 12 to 16 year old students.
e. After everyone has been able to go through each group we
will come back to one large group and allow time for
questions and tell them that they need to be practicing their
brushing skills at home. Brushing for at least 2 minutes and
making sure that they are removing all the plaque from their
teeth. We will briefly tell them what we will be discussing
next week.

help to ensure that the teeth do not hit each other and chip or
break. This also will help keep the jaw in alignment should
they get hit too hard.
c. Gingivitis or healthy tissue- Tissues should be a light pink
color, that is not puffy, and they should not bleed while
brushing and flossing.

guards, and Caitlins pod will be discussing what healthy
tissue looks like.

a. Fluoride- Is a natural occurring mineral that you can get from
many different sources. It is stored in the bones and teeth.
This helps to strengthen the enamel. It also helps to fight
against cavities. Fluoride is found in tap water, toothpaste
and mouth rinses.
b. Sealants are a tooth colored composite material that when
placed correctly can help protect the teeth from caries.
c. Modify brushing technique if needed to more effectively
remove plaque
d. Plaque score- We will have the students brush their teeth and
then disclose each student and record on the plaque sheet
where the student is missing while brushing.
e. Posttest- We will use the same test that we used for during
the pre-test. We will have each student take the test so that
we can compare the results to that of the pre-test. This will
show if the students have made any improvements to their
dental health knowledge.
f. We will play the fluoride vs bacteria game using the large
tennis balls found at the academy. Like dodge ball, there will
be two teams one side is fluoride and the other is bacteria.

i. Fluoride is a natural occurring mineral that you can get from
many different sources. It is stored in the bones and teeth.
This helps to strengthen the enamel. It also helps to fight
against cavities. Fluoride is found in tap water, toothpaste
and mouth rinses.
j. Sealants are a tooth colored composite that when placed
correctly can help to protect the tooth surface from caries.
The sealants can absorb the fluoride from your water and
further increases your teeths chances against caries.
k. At this time we are going to hand out a posttest for all of you
to take. This will let us know how much you have learned
from our program.
l. After the posttest we will take a plaque score on each
participant and modify their brushing technique if needed.
m. Ricky, Jennifer, and Caitlin will conduct plaque scores to help
evaluate the effectiveness of the program, and to allow the
students see how effective their brushing techniques are at
removing plaque.
n. We will play the fluoride vs. bacteria game. We will divide the
students up into two teams; fluoride and bacteria. Using the
large tennis balls we will have a dodge ball game in which we
can illustrate how the fluoride helps to protect the teeth from
caries.

3. CONTENT OUTLINE
a. Fluoridated water. We will discuss the need for fluoridated
water and that it can comes from tap water. Bottled water
may contain fluoride but you must read the label. The
fluoride is important to bones and teeth. It helps to
remineralize the teeth and can help fight against cavities.
b. Plaque: We will show the students a flip chart on the
different stages of plaque. What it looks like and where it
forms on the teeth.
c. Caries Process: We will show a flip chart showing how the
process begins and progresses. We will show them a chart
on how much sugar is in each type of drink available.
d. Sugar content of sports and energy drinks. We will show how
much sugar is actually in the sports and energy drinks. And
we will discuss that drinking them over extended periods of
time is more harmful to the teeth then drinking it all at once.
e. Save a tooth kit. This is a kit that helps keep the tooth from
being contaminated should the tooth become knocked out of
the students mouth. We will discuss the importance of not
rinsing the tooth. But either placing it back in the mouth
where it belong and holding gently pressure on it but being
careful not to touch the root. The tooth can also be placed in
milk or the save a tooth kit. The student should immediately
go to the dentist for re-implantation of the tooth.

Technology in the Dental Hygiene program.
g. Today we are going to be talking about the benefits and the
importance of fluoridated water, not having sugary drinks,
what plaque is and how the caries process begins, and the
importance of having a save a tooth kit on site.
h. Plaque: We will show the students a flip chart on the
different stages of plaque. What it looks like and where it
forms on the teeth.
i. Caries Process: We will show a flip chart showing how the
process begins and progresses. The caries process starts
from the time of sugar consumption and continues to
demineralize the tooth enamel as the pH level in the mouth
drops. This eventually can lead to a large carious lesion.
j. At this time we will show a chart that shows the amount of
sugar found in sports drinks and energy drinks. This helped
them to visualize how much sugar they were in taking over a
period of time. It is important that they understand that it is
more harmful to their teeth to sip on these types of drinks.

Part II Evaluation

Program Design: We planned for 20 students to participate in the oral health
program however only 10 showed up. The program went according to plan. The
program was promoted with the use of the constant contact email that is sent out
weekly as well as verbally reminding the parents and students after their daily
lessons. We planned for four weekly lessons lasting 45 minutes. We split the group
into small learning sessions to better give one on one attention.
Program Objectives:
1. To increase the basic dental knowledge by 50%, measured by a pre and post-
test.
2. Increase the utilization of oral protective equipment to 100%. This will be
measured the site implementing a mandatory use of oral protective
equipment by the students during sparing.
3. To reduce the plaque score by 15% measured by a pre and post plaque score.
4. To set an age limit on the purchase of sugary drinks measured by
implementation of this policy.
5. To purchase a Save-A-Tooth kit (or equivalent) to have on site in case of
emergencies, to be measured by a follow up call to the TaeKwon Do academy.
We did a pre-test to show their oral health learning level and plaque score. The
mean score for the learning level pretest was 54% . The mean post score for the
learning level was 83%. This shows that our group did meet our expectations for
increasing their dental knowledge by 54%. The TaeKwonDo school now implements
a mandatory mouth guard usage during all contact skills and sparring. The mean
plaque score prior to any education was 1.7 and the mean plaque score after
education was 1.0. The pre and posttest means for plaque score shows that the
participants were able to reduce their plaque score by 40% We however not able to
put an age limit of the sale of sugary drinks, we did make the owner aware of the
effects that they have on the mouth. He decided that it should be up to the parents
discretion as to what their children drink. We were able to get the TaeKwonDo
school a save a tooth kit to have in case of emergency. The staff has been trained on
what to do in that situation.
Results: The participants were able during the questions and answer discussion to
describe what plaque was, how to brush effectively and how caries form. The
objectives were met. The participants were able to describe plaque in their own
words, demonstrate proper brushing technique on them, and explain step by step
how a cavity forms.
Evaluation: The strength of this program was the size of the groups; with the
smaller group size we were able to have a more one on one approach in meeting the
needs of the participants. The children loved the puppet and were able to make
brushing more fun. The weakness of the program was the attention span of the
children. The participants had to learn that first they were expected to come in and
listen and then we would have fun, instead of being there for TaeKwonDo where
they are constantly moving around. Recommendations for the program would have
been more advertisement and changing to a later time. We were competing with
church activities and being at 4:00 when children just got out of school made it
difficult for the parents to get the students there.
Future site for oral health program: The site was in excellent condition; the floor
space was large enough for us to set up multiple groups at a time. They provided us
with tables, and anything else that we needed. The staff was courteous at all times
and was very helpful. The time of the program was difficult for the students to get to
and being on Wednesday some had previous commitments.
Learning Value: The pre-test stated that the learning value of the students was
unaware. After the first lesson was complete their learning value had already
increased tremendously. After completing this project Caitlin learned that she is
really good at working and teaching small children. Ricky learned that he likes to
develop and implement oral health programs. Jennifer learned that her teaching
style for the oral health program differed than that of her TaeKwonDo teaching
style.
Collaboration: Caitlin, Ricky, and Jennifer collaborated on this lesson. We each took
an objective and made sure it was met. We trusted in each other that we would get
the job done. Each member was able to bring a skill set to the project that was
unique and ideal for the program. We each were able to feed off of each others skills
to make the program a success.
