Observation Directions If the specified behavior is observed, the observer should mark the box next to the indicator. If the behavior is not observed, no mark should be made next to the indicator. The observer should document the rationale for the findings in the space provided. Upon completion of the observation, the observer will review the document with the intern/clinical teacher. Please sign the form using a digital signature. The mentor, cooperating teacher or campus administrator may email the observation to the candidate support team. The field supervisor will provide a copy of the to the intern/clinical teacher and campus administrator. The field supervisor is required to enter documentation of the observation into the Region 4 database and upload the signed form.
