*Access, administrative, and third party pass through fees charged by the
government to access public records may be charged for these additional
services.

Please mail your request specifying the package and/or additional services, the
completed and signed Schedule F Release form(s) by each volunteer and/or staff
person, and the appropriate payment for services to the Office of Ministerial Concerns,
Attn: Susan Padgett, 1808 West End Avenue, Suite 501, Nashville, TN 37203-2581.
Make your check payable to Tennessee Conference. Your request will not be
processed without the appropriate payment.

The following are forms you may need in processing your request to Susan Padgett.

requesting a background check.
