1) This form is to be completed on every second month from the time when the Cadet joined the vessel.
Additionally, it should be completed when the Cadet leaves the vessel.
2) Completed report must be returned to the Personnel Department with the monthly returns. The report must include
as much information as is practicable to cover all aspects of the Cadets performance on board.
3) A negative or poor report in any category should be discussed with the Cadet to give him the opportunity to take
corrective action. Please indicate in Masters comments which points have been discussed.
