Informed Consent
I volunteer to participate in a research project conducted by the University of San JoseRecoletos College of Nursing Level IV Block 1 students. I understand that the project entitled, Philippine Nurses Licensure Examination: A lived Experience, is designed in order to assess the psychological, behavioural, environmental and personal effects of failing. My participation in this project is voluntary. I understand that I will not be paid for my participation. I may withdraw and discontinue participation at any time without penalty. If I decline to participate or withdraw from the study I will not be pressured. The interview will approximately last for 30- 45 minutes. Notes will be taken, a voice recorder for recording purposes and subsequent dialogue will be made during the interview. If I feel uncomfortable in any way during the interview session, I have the right to decline to answer any question or to end the interview. If I dont want to be voice recorded I will not be able to participate in the study. I understand that the researcher will not use my name in any reports using information obtained from this interview and that, my confidentiality as an informant in this study will remain secure. I understand that this research study has been reviewed and approved by the Dean of the University of San Jose- Recoletos, College of Nursing. I have read and understand the explanation provided to me. I have had all my questions answered to my satisfaction, and I voluntarily agree to participate in this study. I have been given a copy of this consent form.
