Assessment Blueprint for _Family History Narrative (Formal, Summative Assessment)__

Standard (use notation) Learning Targets Data I will collectFormat of question, question #s All details from worksheet included in book At least five pages; at least one complete sentence on each page that describes ideas, thoughts, and feelings; detailed illustration on each page that clarifies ideas, thoughts, and feelings; logical order of details (beginning, middle, and end, with middle details in logical order) How data will be organized and how the teacher will use the information collected Data will be scored on the rubric. Each individual student will have a separate rubric on which his or her scores and feedback will be recorded. I will use the information to provide examples and feedback to students, their families, and other stakeholders about the students progress and abilities. Because this is a performance assessment, I will also display the books in the classroom so that students may read them during Quiet Free Choice or Read to Self time. Plan for students using information collected I will use the information the next time we do a writing project because I will know what each students strengths and weaknesses are ahead of time and will therefore be prepared to provide more guidance to each student in the necessary areas. Students will use the feedback and guidance to improve their writing and investigation skills as we progress through first grade.

ANALYSIS: A students score of fifteen to twenty on this rubric indicates proficiency in the first two objectives. Including at least f ive pages in the book allows for a clear beginning and end to the narrative with at least three details in the middle. Including at least one complete sentence and one detailed illustration on each page which detail and clarify the students ideas, thoughts, and/or feeling indicates proficiency in the second objective. Having the books pages in logical order demonstrates proficiency in taking the information found from the investigation and turning it into a narrative (which is stated in the first standard and included in the second objective). Including all or most of the details from the Family History worksheet indicates that investigation into the family history has taken place and been processed by the student (first objective).

An informal, summative assessment will also be included. This takes place in the form of the students presentations. I will informally assess wheth er each student is describing (through reading his or her book aloud) his or her thoughts and feelings about the topic. This assessment addresses the third objective.
