APPOINTMENT LETTER
This agreement is made on the 4th of May 2013 between Saaol Heart Center and Miss Krupali Jasani aged about 24 yrs residing at Katargam, Surat. (Herein after referred to as Medical officer at our Center) of the one part and Saaol Heart Center having its registered office at Lajpat Nagar New Delhi of the second part. This agreement has been entered into between the Saaol Heart Center Surat and Miss Krupali Jasani and will be effective from 4th May 2013 for a period of ONE MONTH i.e. up to <end date>. This agreement supersedes all previous agreements. The Agreement work a monthly renewal bases, so in future if you want to leave job you have to inform company fully one month before. Not to follow this rules will breach regulation will leads to further step decide by head office. Now this agreement witnesses the following terms and conditions agreed between the parties and are binding on both. Employees shall be engaged under the designation of Medical Officer at Saaol Heart Centre based at above address. You will work on all working days during the tenure of the agreement. Company shall be paid remuneration of Rs.16000 (Sixteen thousand rupees only) per month including all succeeding 4th of the calendar month during the agreement period.
