Issuing of Identity cards to Disabled Persons 1. Application Form (Form SW-13) 2. Application Form complete in all respect alongwith following self attested documents: Two passport size photographs. Residence Proof for the last three years duly attested (Voter Idgrentity Card, Ration Card, Electricity Bill) Proof of the age (Birth Certificate/ Voter Identity Card/ School Certificate). 3. The applicant is deputed to the PMO, General Hospital, Sector 16 for medical examination by the Medical Board to assess the PPD of the applicant. 4. On receipt of the recommendations received from the PMO , Identity Cards are issued to those applicants who have attained 40% or above permanent disability. 5. Applications are considered on first-cum-first serve basis. Eligibility The Social Welfare Department is issuing Identity Cards to the disabled persons who are residents of Chandigarh. Proof of residence, age and two attested passport size photographs are required to be submitted with the application. Following facilities are available to the holders of disability identity cards:(i) This card is valid for a period of five years from the date of issue, for the persons below 18 years of age. (ii) This card is permanent for those above 18 years. (iii) This is not transferrable and should be kept and used by the persons in whose name it is issued. (iv) In case the card is lost, a new card will be issued on production of record. (v) 100% concession to blind persons for travelling in CTU buses in Chandigarh and 50% concession to other disabled persons.
