LIST OF DOCUMENTS NEEDED BEFORE IP APPLICATION PACKET CAN


BE SENT TO CHI
Your IP (Internship or Training Program) Application Packet includes the following:
1. Application Packet Cover
2. Intern Profile
3. Professional Focus, Goals and Objectives Sheet
4. Authorization and Release Agreements (please initial beside each one)
5. Internship Policy Addendum

THE FOLLOWING SUPPORTING DOCUMENTATION IS REQUIRED before IP application


packet is complete and can be sent to CHI

6. Four (4) photographs – 2 passport-size and 2 professional demeanor photos


7. Letter of Introduction to the Host Business
8. Curriculum Vitae / Resume
9. Two (2) Letters of Recommendation
10. Proof of University Attendance or Diploma or Current Enrollment Certificate
11. Proof of Medical Insurance* and Policy Number (see minimal requirements below)
12. Proof of level of English from Recognized English Language Test or Signed Documentation
from an Academic Institution or English Language School
13. Payment in full for Work Experience Program

All documents must be ORIGINALS (faxed or emailed copies are not acceptable). These must be
received by CHI 12 weeks in advance of the planned departure for the US. Note that LSI needs to
receive the applications more than 12 weeks in advance to be able to assess the application and
forward on to CHI for placement.

*Insurance Requirements
ƒ The policy must provide medical benefits of at least $50,000 for each
accident or illness & at least $7,500 in repatriation benefits.
ƒ The policy must pay up to $10,000 for the expenses of travel in cases of a
serious illness or injury
ƒ Health Insurance policy may establish a waiting period before it covers
pre-existing conditions as long as the waiting period is reasonable by
current standards in the insurance business.

If you elect to satisfy the insurance requirement through a policy issued in your home country,
the policy must be backed by the full faith and credit of your government. Otherwise, the
company providing the insurance must meet minimum rating requirements
established by Department of State (an A.M. Best rating of "A-" or better, an Insurance
Solvency International, Ltd. (ISI) rating of "A-1" or better, a Standard & Poor's Claims-
Paying Ability rating of "A-" or better, or a Weiss Research, Inc. rating of "B+" or
