DAILY CONTROL OF FITNESS AT WORK ATLANTIS COMPANY PRACTICE A ZERO TOLERANCE, WITH REGARDS TO MISUSE OF ALCOHOL AND GRUG RELATED SUBSTANCES. SUBSTANCES ALL EMPLOYEES SIGNED A EMPLOYMENT CONTRACT WITH ENGAMENT CLEARLY SPECIFYING THE COMPANIES STANDARD TO ALCOHOL AND SUBSTANCE ABUSE. EACH EMPLOYEE DOES HAVE THE OPPORTUNITY TO DECLARE HIS/HER CONDITION BEFORE REPORTING FOR DUTY, WITH NO CONSEQUENCES IF DECLARED BEFORE TESTING. DAILY BREATHALYZER CHECKS ARE CONDUCTED ON ALL EMPLOYEES DURING THE START OF EACH SHIFT AND RECORDED ON ATTENDANCE REGISTER. A OVER THE LIMIT READING WILL LEAD TO A DISCIPLINARY HEARING AND DISMISSAL AS CLEARLY STATED IN THE COMPANIES POLICIES AND PROCEDURE MANUAL. SUPERVISORY PERSONAL MUST KNOW THEIR WORKERS AND ABNORMAL BEHAVIOR SHOULD BE RECOGNIZED AND ADDRESSED DURING THE 5 MINUTE SAFETY TALK.

SAFETY MEETINGS MUST BE CONDUCTED BY SUPERVISORS DAILY PRIOR TO COMMENCEMENT OF WORK. EMPLOYEES UNDER THE INFLUENCE OF ALCHOL OR DRUGS ARE NOT ALLOWED TO PROCEED TO WORK- BEHAVIOUR SAFETY. WORK STANDARDS MUST BE COMPLIED WITH AT ALL TIMES. THE REQUIRED PPE MUST BE WORN AT ALL TIMES. NO WORKER MAY BE UTILIZED TO PERFORM TASKS FOR WHICH THEY HAVE NOT BEEN TRAINED, FOUND COMPETENT AND APPOINTED. INEXPERIENCED TRAINED WORKERS MUST CARRY OUT TASKS UNDER THE SUPERVISION OF AN EXPERIENCED WORKER. ALL INJURIES MUST BE REPORTED IMMEDIATELY TO THE RESPONSIBLE SUPERVISOR. SUPERVISOR MUST INVESTIGATE EACH INJURY OR INCIDENT IMMEDIATELY. SUPERVISORS MUST COMPLETE AN INJURY REPORT FORM AND HAND A COPY TO THE RESPONSIBLE CLIENT AND COMPANY SAFETY OFFICER INJURIES AND OTHER DOWNGRADING INCIDENTS MUST BE REPORTED TO THE RESPONSIBLE MINING OFFICIAL AND COMPANY SAFETY OFFICER IMMEDIATELY. INJURIES AND SERIOUS INCIDENTS MUST BE INVESTIGATED AND REPORTED TO THE SHEQ MANAGER IMMEDIATELY. MONTHLY FIRE DRILLS MUST BE CONDUCTED. NEW CONTRACTOR PERSONNEL MUST BE PROPERLY INDUCTED AND FAMILIARISED WITH HAZARDS AND RISKS PERTAINING TO THE TASK AND THE WORKING INVIRONMENT.

CONTINUOUS RISK ASSESSMENT OF THE WORKING ENVIRONMENT MUST BE CONDUCTED(HIRA). COPIES OF HAZARDS MUST BE FOREWARDED TO THE SAFETY OFFICER AND SHEQ MANAGER. THE CLIENTS VERTICAL AND HORIZONTAL STANDARDS MUST BE COMPLIED WITH. THE CLIENTS LIFTING STANDARDS MUST BE COMPLIED WITH. T-SPRAGS, STOP SIGNS AND/ORBARRICADES MUST BE ERECTED TO PROTECT WORKERS AGAINST MOVING RAILBOUND, TRACKLESS AND MOVING MACHINERY. WORK PLACES MUST BE TESTED AND INSPECTED FOR FLAMMABLE GAS. GAS PLACES IN ACCESS OF 32.5DEGREES WET BULB MAY NOT BE WORKED IN, REPORT TO RESPONSIBLE MINE SUPERVISOR TO TAKE ACTION. HANGING, SIDEWALLS AND FOOTWALLS MUST BE EXAMINED, MADE SAFE AND SUPPORTED TO CLIENT STANDARDS BEFORE ANY WORK IS CONDUCTED IN THE AREA. AREA ALL MACHINERY, TOOLS AND EQUIPMENT MUST BE INSPECTED PRIOR TO USE AND THE APPROPRIATE CHECKLIST COMPLETE. DAMAGED MACHINERY, TOOLS OR EQUIPMENT MAY NOT BE USED. MACHINERY AND EQUIPMENT MUST BE MAINTAINED TO COMPANY STANDARD. STANDARD

RISK ASSESSMENT STANDARD PROCEDURE


ATLANTIS STANDARD PROCEDURE IS REGULARLY UPDATED AND REVISED TO ENSURE PROPER CONTROL OF ALL ACTIVITIES. CONTINUOUS REVISING OF BASE LINE RISK ASSESSMENTS, TO INCLUDE PREVIOUS ARISING INCIDENTS AND ACCIDENTS, TO ILLUMINATE RE-OCCURRENCE OF EVENTS. WE MAKE USE OF THE KISS (KEEP IT SPECIFIC AND SIMPLE) APPROACH TO ISSUE BASE LINE RISK ASSESSMENT. ALL RELEVANT CREW MEMBERS MUST BE INVOLVED DURING THE SITE SPECIFIC RISK ASSESSMENT PROCESS, BY KEEPING IT SIMPLE EVEN THE LOWEST RANKED EMPLOYEE INVOLVED WITH THE TASK UNDERSTANDS THE RISKS INVOLVED. WITH THE SPECIFIC TASK TO BE CONDUCTED, AND KNOW HOW TO ENSURE HIS OWN AND FOLLOW WORKERS SAFETY COMES FIRST. TO SUMMARIZE ABOVE ATLANTIS STRIVE TO UTILIZE RISK ASSESSMENT, TO ITS FULL POTENTIAL, THIS MEANS THAT RISK ASSESSMENTS MUST BE A LIVE DOCUMENT USED BY ALL EMPLOYEES ON A DAILY BASIS TO ELIMINATE OR MINIMIZE RISKS OF FATALITIES, , INJURIES AND INCIDENTS ARISING. NOT A LEGAL DOCUMENT FILLED IN THE SAFETY OFFICER SHE FILE.
